小卵泡排卵不孕症患者的临床特征及妊娠结局

摘    要:目的探讨小卵泡排卵不孕患者的临床特征及妊娠结局。方法用回顾性方法对73例经阴道B超监测发现为自然周期小卵泡排卵的不孕症患者临床表现及妊娠结局进行分析。结果 不孕妇女中小卵泡排卵的发生率为16.7%(73/437),其中21.9%的患者有自然流产史,明显高于其他不孕患者,53.4%的患者表现黄体功能不足,35.6%诊断为不明原因不孕。小卵泡排卵后的妊娠率为11.3%,但自然流产率高达80.0%。结论 小卵泡排卵可能是引起不孕的原因之一,多被诊断为黄体功能不全或不明原因不孕,患者可有自然流产史。经阴道B超监测卵泡发育是诊断的主要方法,因自然流产率高,建议小卵泡排卵周期取消治疗计划。

Abstract:Objective To investigate the clinical features and pregnant outcomes of infertile women ovulated with small follicle (OSF) . Methods Transvaginal B - ultrasonography was used for infertile women to monitor the follicular development during nature cycles, then the date of clinical features and pregnant outcomes of the patient of OSF were analyzed retrospectively. Results Among 437 infertile women, there were 73 (16.7%) with OSF. Among OSF cases, 21.9% had history of spontaneous abortion, 53.4% showed luteal phase deficiency (LPD) and unexplained infertility were diagnosed in 35.6%. The pregnant rate with OSF was 11.3% , whereas the incidence of spontaneous abortion was up to 80.0% . Conclusions OSF might be a cause of infertility. Patients often had been diagnosed as LPD or unexplained infertility and had spontaneous abortion history. Transvaginal ultrosonography is a major diagnostic method. Since there is higher rate of spontaneous abortion, it is suggested to cancel the treatment cycle when OSF has been found.
