大剂量低频率使用重组人促红细胞生成素临床对照研究

目的比较重组人促红细胞生成素每周1次低频率使用以与每周3次高频率使用的效果、安全性。方法维持性血液透析患者60例随机分为两组：实验组30例,采用促红素10000iu/支,皮下注射,1次/周治疗;对照组30例,采用促红素3000Iu/支,皮下注射,3次/周治疗。两组患者治疗间期均为12周。治疗期间观察血常规、血清肝肾功能、电解质、血脂、血压、脉搏、铁蛋白、转铁蛋白饱和度的变化及不良反应。结果实验组总有效率86.67%,对照组总有效率83.33%,两组比较差异无显著意义;治疗前后白细胞、血小板、血脂、肝肾功能、电解质无明显变化;两组不良反应的发生差异无显著意义。结论低频率促红细胞生成素每周1次与高频率促红细胞生成素每周3次治疗肾性贫血的疗效和不良反应,差异无显著性,但减少了注射次数,提高了患者的依从性。

The clinical comparative study on use of high-dosage and low-frequency recombinant human erythroietin

Objective To compare the effects and security of use of recom binant human erythroietin（rHuEPO）between once-weekly,low-frequency and thrice-weekly,high-frequency.Methods 60 cases with maintenance dialysis patients were randomly divided into 2 groups.30 patients in the experimental group accepted 10000iu r-HuEpo by subcutaneous injection once a week and 30 patients in the control group accepted 3000iu r-HuEpo by subcutaneous injection thrice a week for 12 weeks.During the period,the change of blood routine,the function of liver and kidney,electrolyte,plasma lipid,blood pressure,pulse,serum ferritin,transferrin satura-tion and side effects were observed.Results The effective rate of the experimental group was 86.67% and that of the control group was 83.33%,There were no significant differences in 2 groups.White cells,platelets,plasma lipid,the function of liver and kidney,electrolyte have no obvious changes,side effects showed no statistical difference in two groups.Conclusion There were no significant differences in therapeutic effects and side effects on use of r-HuEpo between once-weekly,low-frequency and thrice-weekly high-frequency in the treatment of renal anemia,but it decreased injection frequency,and increased patient＇s compliance.
