囊内分隔在超声鉴别诊断复杂性肾囊肿中的应用价值

目的探讨囊内分隔数目及厚度在超声鉴别诊断复杂性肾囊性病灶良恶性中的价值。方法回顾性分析70例(79个病灶)复杂性肾囊性病灶的超声表现,评价所有病灶的分隔数目和厚度,并将诊断结果与"金标准"(病理结果、CT检查、超声随访≥6个月)进行对照,用ROC曲线筛查特异性指标并确定界值。结果 ROC曲线显示分隔厚度对鉴别诊断复杂性肾囊性病灶的良恶性有很高的诊断价值(曲线下面积>0.9);囊内分隔数目对鉴别诊断复杂性肾囊性病灶良恶性有中等诊断价值(曲线下面积>0.7)。囊内分隔数>4条和分隔厚度>1.5mm是鉴别诊断复杂性肾囊性病灶良恶性的最佳界值。结论囊内分隔数目和厚度在鉴别肾复杂性囊肿的良恶性方面有诊断价值,囊肿分隔越多、越厚则恶性的可能性越大。

Usefulness of septa in differentiation of complex renal cyst by Ultrasonography

Objective To evaluate the number and thickness of septa in differential diagnosis of complex renal cysts using Ultrasound.Methods The features of septa in 70 cases(79 complex renal cysts) were retrospectively analyzed.All the data were compared with ’gold standard’(pathology,CT images and follow-up ≥ 6 months).The specific diagnosis index and cutoff value were determined by receiver operating characteristic(ROC) curve.Results The thickness of septa had high value(curve area>0.9) in differentiation of the complex renal cysts by the ROC curve,and the number of the septa had middle value(curve area>0.7).The cutoff value of septa′s number was > 4,and of septa′s thickness was >1.5 mm,which was the appropriate cutoff value in differentiation of the complex renal cysts.Conclusion Both of number and thickness of septa are the valuable index in differential diagnosis of complex renal cysts.The more and thicker of septa,the more possibility of malignancy.
