中西医结合治疗糖尿病肾病临床观察

目的：观察自拟补肾活血汤中西医结合治疗糖尿病肾病的临床疗效。方法：对34例糖尿病肾病患者在西药常规治疗的基础上应用自拟补肾活血汤（治疗组），并与单纯西药常规治疗对照组34例进行疗效比较，观察两组疗效。结果：治疗组显效18例，有效11例，无效5例，总有效率为85．29％（95％CI：68．9％～95．1％）；对照组显效8例，有效6例，无效20例，总有效率为41．18％（95％CI=24．7％～59．3％）；两组综合疗效比较（u=3．2736，P=0．0012），差异显著；治疗后，治疗组较对照组24h尿蛋白定量、肌酐均显著下降（P〈0．01），尿素氮水平也明显下降（P〈0．05）。结论：自拟补肾活血汤中西医结合治疗糖尿病肾病患者的综合疗效优于单纯西医治疗的疗效，其收益为OR=0．t2（95％CI：0．04-0．39），NNT=2（95％CI=1．49～3．11）。

Clinical Observation of Integrated Traditional Chinese Medicine and Western Medicine on Diabetic Nephropathy

Objective ： To observe the clinical curative effect of Decoction of Invigorate Kidney to Promote Blood Flow on diabetic nephropathy. Method：34 cases in treatment group were treated by Decoction of Invigorate Kidney to Promote Blood Flow as well as conventional western medical therapy, while 34 ones in control group were done by only conventional western medical therapy. Result：In treatment group 18 cases were excellent, 11 ones effective,5 ones ineffective,and the total effective rate was 85.29% （95% CI =68.9%-95.1% ）. In control group 8 cases were excellent,6 ones effective,20 ones ineffective,and the total effective rate was 41. 18% （95% CI = 24.7%-59.3% ）. There was significant difference between the two groups （u =3.273 6 ,P =0. 001 2）. 24 h urinary protein quantity and creatinine were decreased more markedly in treatment group than those in control group （ P 〈 0.01 ）. And urea nitrogen was the same as them （ P 〈 0.05 ）. Conclusion ：The curative effect of Decoction of Invigorate Kidney to Promote Blood Flow combined with western medicine is better than that of only western medi- cine on diabetic nephropathy, OR = 0.12 （95% CI = 0.04 - 0.39）, NNT = 2 （95 % CI = 1.49 - 3.11 ）.
