老年原发性高血压患者社区强化治疗的效果评价

目的 评价强化治疗模式在社区老年原发性高血压患者管理中的效果. 方法 选取2005年1月-2007年4月建卡且建卡时年龄≥60岁的原发性高血压患者为研究对象,分为强化治疗组(761例)和一般治疗组(390例),对比两组高血压防治知识的知晓率、治疗的依从性以及血压的控制率. 结果 两组建卡入组时基本情况差异无统计学意义(P<0.05).最终随访强化治疗组在高血压防治知识率上高于一般治疗组;其在对药物和非药物治疗的依从性上亦优于一般治疗组;强化治疗组总的血压达标率为60.8%,未达标患者6个月内达标率为29.4%,优于一般治疗组的6.0%,差异有统计学意义(P<0.05). 结论 社区强化治疗有利于提高老年患者对高血压知识的知晓率及对综合治疗措施的依从性,并最终提高高血压的控制率.

Effectiveness of Community-based Intensive Management Mode for the Elderly Patients with Hypertension

Objective To explore the effectiveness of community-based intensive management mode for the elderly patients with hypertension.Methods Totally 1151 elderly patients (≥60 years;548 men and 603 women) with hypertension who were managed in our center from January 2005 to April 2007 were enrolled in this study.Patients were divided into intensive management (IM) group (n=761) and common management (CM) group (n=390) based on their frequency of visiting the community clinics.Their awareness of hypertension-related knowledge,compliance on treatment,and the rate of blood control were compared between these two groups.Results The baseline indicators were not significantly different between these two groups.After several years of intervention,the outcomes in the IC group were significantly superior to those in the CM group,especially the blood pressure control rate (60.8% in the intensive management group).After 6 months of further intervention,more patients in the intensive management group (29.4% of the uncontrolled patients) had their blood pressure well controlled;on the contrary,this rate was only 6.0% in the CM group.Conclusion Community-based intensive management is effective in raising the elderly patients' awareness of hypertension,increasing their compliance on treatment,and ultimately improving the control of blood pressure.
