摘    要:目的探讨不同引导方法经皮肺穿刺活检在肺占位病变诊断中的应用价值。方法对519例肺占位病变患者分别选用CT、B超、模拟定位机三种不同引导方法进行经皮肺穿刺,对其阳性(明确病理诊断)率、病理结果及并发症进行回顾性分析。结果519例患者总阳性率为96.72%,其中CT引导组阳性率96.41%,模拟机引导组阳性率96.53%,B超引导组阳性率97.35%。3组比较无显著性差异。穿刺中发生各种并发症46例,总发生率为8.86%,主要为气胸。结论根据肺占位病变特点选用不同引导方法进行经皮肺穿刺,可获得更高的阳性率,且并发症低、安全可靠。

Abstract:Objective To evaluate the application value of lung biopsy through different method-guided percutaneous paracentesis in the diagnosis of space-occupying lesions of the lung. Methods 519 patients with space-occupying lesions of the lung underwent lung biopsy through CT, B-ultrasonography or simulator-guided percutaneous paracentesis. Retrospective analysis was carried out on the positive rate, pathological diagnosis and complication. Results The total positive rate of 519 cases was 96.72% , of which the positive rates of CT, B-ultrasonography and simulator -guided cases were 96.41% , 96. 53% , and 97. 35%. There was no evident difference among the three methods. During punctures, complications occurred in 46 cases with an occurrence rate of 8. 86% , most of which were pneumothorax. Conclusion According to the characteristics of space-occupying lesions of the lung, different guide methods in percutaneous pneumocentesis biopsy are performed, the positive rate can be higher, and the complications can be fewer.
