1例主动脉夹层高龄孕妇剖宫产并主动脉修复术的麻醉用药分析

1例高龄孕妇因突发主动脉夹层，麻醉医生采用丙泊酚+氯胺酮诱导，辅以低浓度的吸入麻醉行子宫下段剖宫产术，待胎儿娩出后，予咪达唑仑+舒芬太尼+维库溴铵行气管插管后，行升主动脉置换+主动脉弓及降主动脉支架植入术，避免了胎儿呼吸抑制及产妇血流动力学波动，最大程度地保证母婴的安全。

Analysis of Anesthesia for Cesarean Section and Aortic Repair in an Elderly Pregnant Woman with Aortic Dissection

An elderly pregnant woman was hospitalized for emergency treatment due to sudden aortic dissection. Anesthesiologists used propofol + ketamine induction, supplemented by low concentration inhalation anesthesia to perform ''lower uterine cesarean section''. After the delivery of the fetus, the woman received midazolam + sufentanil + vecuronium and tracheal intubation, followed by ''ascending aortic replacement + aortic arch and descending aortic stent implantation''. Through rational drug use, fetal respiratory depression and maternal hemodynamic fluctuations were avoided, and maternal and infant safety was guaranteed to the greatest extent.
