果胶铋四联和三联疗法治疗幽门螺杆菌阳性十二指肠溃疡

目的:对比果胶铋四联(果胶铋、奥美拉唑、阿莫西林、甲硝唑)一周疗法和三联(果胶铋、阿莫西林、甲硝唑)两周疗法对确诊幽门螺杆菌(Hp)阳性的十二指肠溃疡的疗效。方法:91例随机分为四联疗法组(46例)和三联疗法组(45例)治疗,胃镜检查和组织活检于服药前及停药4周后进行。结果:四联疗法组溃疡愈合率为85%,Hp根除率为83%,不良反应发生率为24%;而三联疗法组分别为75%、82%和20%,两组比较无显著性差异。结论:果胶铋四联和三联疗法均对幽门螺杆菌阳性的十二指肠溃疡有效,而四联一周疗法疗程短、价格较便宜,可优先选用。

The Effect of Bismuth Pectin Quadruple and Triple Therapy on Helicobacter Pylori Positive Duodenal Ulcer

Objective:To compare the effect of one-week bismuth pectin quadruple therapy(bismuth pectin,omeprazole,amoxicillin,and metronidazole)and two-week triple therapy(bismuth pectin,amoxicillin,and metronidazole)on confirmed Helicobacter pylori(Hp)positive duodenal ulcer.Methods:91cases were randomly assigned to quadruple(n=46),or triple therapy groups(n=45).Endoscopy with biopsy was performed before and 4 weeks after stopping medicines.Results:The ulcer healing rate,Hp eradication rate and incidence of mild adverse effects were 85%,83% and 24%,respectively in quadruple therapy groups,and 75%,82% and 20% respectively in triple therapy groups.There was no significant difference between the two groups.Conclusion:Both bismuth quadruple and triple therapy are effective in the treatment of Hp positive duodenal ulcer,but quadruple therapy has a shorter course and cheaper price.
