LeVeen电极经皮肺穿刺射频消融对肺部多发结节性肿瘤治疗的CT观察

目的 探讨LeVeen电极经皮肺穿刺射频消融治疗肺部多发结节性肿瘤的疗效及CT影像学特征。方法 在CT引导下经皮肺穿刺，使用LeVeen电极对53例肺多发结节性肿瘤，共79个结节进行了射频消融治疗，采用CT对治疗过程进行观察及术后复查。结果 治疗过程中CT表现有；病灶周围模糊斑片影（40/53）；病灶体积相对增大（31/53）；病灶内点状低密度区及密度减低区（30/53），穿刺针道周围模糊斑片影（8/53），对45例进行了随访复查，随访时间≥3个月的共23例，病灶体积明显缩小的占18例，结论 LeVeen电极经皮肺穿刺射频消融治疗肺部多发结节性肿瘤是一种安全，有确切疗效的方法。应用于肺部转移性肿瘤的治疗将会是一种很有希望的治疗方法。

CT observation on percutaneous radiofrequency ablation with LeVeen electrodes for lung multi-focal nodular malignancies

Objective To investigate the efficacy and its CT features during radiofrequency ablation (RFA) treatment of lung multi focal nodular tumors with LeVeen electrodes. Methods Under the guide of CT fluoroscopy, the LeVeen electrodes were used in the procedures of percutaneous RFA. This study consisted of 79 lung nodules in 53 patients. After the treatment, CT scanning was performed to observe the CT features in 45 cases within 3 to 221 d. Results The CT features during the treatment included edema around the lesion (40 in 53 patients), enlargement of the lesions (31 in 53 patients), heterogeneous attenuation area in the masses (30 in 53 patients), hemorrhage of the puncture path (8 in 53 patients). In follow up examination, 18 patients in 23 with over 3 months observation showed remarkable decrease in tumor size. Conclusion CT guided percutaneous radiofrequency ablation of multi focal metastastic lung malignancies with LeVeen electrodes is a safe and efficient way of multiple metastastic lung nodes with no indication for surgical resection.
