CEA、CA19-9及CA72-4对消化道癌诊断价值的评价

目的 评价血清肿瘤标志物CEA、CA19-9及CA72-4对食管癌、胃癌及大肠癌的诊断价值。方法 采用放射免疫量度分析法测定68例消化道癌及54例消化道良性疾病患者血清CEA、CA19-9及CA72-4水平。结果 血清CEA在胃癌阳性表达率为56．0％，大肠癌的阳性表达率为59．1％，在食管癌及消化道良性疾病的阳性率仅分别为4．8％和1．9％；CA19.9在胃癌及大肠癌阳性表达率分别为60．0％和27．3％，而在食管癌和消化道良性疾病的阳性率分别为9．5％和9．3％；CA72-4在胃癌及大肠癌阳性表达率分别为52．0％和59．1％，而在食管癌和消化道良性疾病的阳性率为4．8％和1．9％。结论 血清CEA、CA19-9、CA72-4对鉴别胃肠道良恶性疾病有较大价值，联合检测比单项检测准确性更高。

Diagnostic value of combined detection of CEA,CA19-9 and CA72-4 in digestive tract cancer

Objective To evaluate the diagnostic value of t hree serum tumor markers for digestive tract cancer. Methods Th e IRMA were used to measure the serum levels of three tumor markers: carcinoembr yomic antigen(CEA),Carbohydrate antigen 19-9 (CA19-9) and carbohydrate anti gen 72-4(CA72-4) in 68 patients with digestive tract cancer and 54 patients wi th benign digestive tract disease. Results In patients with gast ric cancer and colorectal cancer,the positive rates of serum CEA were 56.0% an d 59.1% respectively,and in patients with esoph- ageal cancer and with benign dige stive disease,the positive rates of serum CEA were 4.8% and 1.9% respectivel y.In patients with gastric cancer and colorectal cancer,the positive rates of s erum CA19-9 were 60.0% and 27.3% respectively,and in patients with esophagea l cancer and with benign digestive disease,the positive rates were 9.5% and 9 .3% respectively. In patients with gastric cancer and colorectal cancer, the p ositive rates of serum CA72-4 were 52.0% and 59.1% respectively,and in pati ents with esophageal cancer and with benign digestive disease,the positive rat es of serum CA72-4 were 4.8% and 1.9% respectively. Conclusion The detection o f serum CEA CA19-9 and CA72-4 has great value in differentiating benign gastro enteric disease from malignant one, and the combined detection of above-mentio ned indexes has better accuracy than that of the single index.
