5-氨基酮戊酸光动力疗法治疗扁平疣的临床疗效观察

目的观察5-氨基酮戊酸光动力(5-aminolevulinic acid photodynamic therapy,ALA-PDT)疗法治疗扁平疣的临床疗效。方法扁平疣患者112例,分为治疗组52例,采用5-ALA-PDT治疗,应用LED光源照射,功率密度70 mW/cm2,距离20 cm,照射20 min。对照组60例,采用冷冻治疗。末次治疗后20 d判定两组疗效。末次治疗后3个月后判定两组复发率。结果治疗组患者共治疗1～4次后,痊愈率92.0%,治疗后无感染、溃烂或瘢痕等不良反应发生。对照组经1～4次治疗后痊愈率71.4%。治疗组有效率高于对照组,差异有显著意义(P<0.05)。随访3个月后,治疗组复发率6.0%,对照组复发率21.4%,治疗组复发率低于对照组,差异有显著意义(P<0.05)。结论 5-ALA-PDT治疗扁平疣安全、有效、复发率低,有待于临床的进一步应用和推广。

Efficacy of 5-Aminolevulinic Acid Photodynamic Therapy on Flat Warts

Objective To observe the clinical efficacy and safety of 5-aminolevulinic acid photodynamic therapy in treating flat warts. Methodr Altogether 112 patients with flat warts were divided into two groups： the treatment group and the control group, consisting of 52 patients and 60 patients respectively. While the patients in the control group being treated with cryotherapy, those in the treatment group were given 5-ALA-PDT with the LED irradiation of a power density of 70 roW/era2 at a distance of 20 cm, which lasted for 20 mi- nutes each time. The clinical effects of the two groups were evaluated 20 days after the last treatment was given, and the recurrence rate were evaluated 3 months after that. Results After 1 to 4 times of treatment, all the lesions of the patients in the treatment group disappeared, and no infection, ulcera- tion, scmring nor other adverse reactions occurred. While the cure rate of the treatment group being 92.0%, the rate of the control group after 1 to 4 times of treatment was 71.4%, indicating that the efficacious rate of the ALA treatment group was significantly higher than that of the control group （P 〈 0. 05 ）. The 3 months follow-up showed that the recurrence rate of the treatment group was signifi- cantly lower than that of the control group （P 〈 0. 05）.Conclusions Featuring a low recurrence rate, ALA-PDT is a safe and effective way to treat fiat warts.
