关于血管内亚低温治疗重型颅脑创伤临床研究

摘    要:目的 对血管内降温治疗急性重型颅脑创伤患者的安全性及有效性进行前瞻性的研究.方法 共30例患者,均于伤后12 h内行血管内降温治疗,体内温度控制在33℃~35℃之间,持续4~7 d平均(21.5±13.7)h.患者均于伤后6个月时根据GOS评估法判定疗效.结果 经6个月随访,良好11例,中残5例,重残6例,植物生存6例,死亡2例、本组患者没有出现与血管内降温系统相关的严重并发症.结论 血管内降温具有降温速度快、目标温度维持稳定、波动性小以及复温速度容易控制等优点.对于重型颅脑损伤的患者是一种安全、有效的治疗方法.

Abstract:Objective To study the security and validity of intravascular cooling system in the treatment of severe traumatic brain injury.Method Thirty adult patients were included in this clinical trial of intravascular mild hypothermia within 12 h after traumatic brain injury.The target temperature was 33℃-35℃,for 4-7 days,average(121.5±13.7)h.According to Glasgow Outcome Scale(GOS),the prognosis of patients was evaluated after 6 months.Results The outcome was good in 11 patients,incomplete recovery in 5,heavy incomplete recovery in 6,vegetable comdition in 6,death in 2.There was no adverse events related to intravascular cooling method.Conclusions High speed and stable control of temperature decline are characteristics of intravascular cooling system.Intravascular hypothermia is safe and effective for prevention and treatment of further brain damage of patients with severe brain injury.
