髓母细胞瘤的显微手术治疗

目的　分析髓母细胞瘤的临床特点、治疗效果及影响预后的因素。方法　分析 2 0例经手术和病理证实的髓母细胞瘤病例的临床表现、诊断及综合治疗方法和影响预后因素 ,重点为手术等治疗与预后的关系。结果　均采用显微手术 ,肿瘤全切 17例 ,次全切 3例 ,手术死亡率为 0 ,19例行手术后放疗、个别化疗并随访 18例 ,获得良好疗效。本组患者术后 2年原位复发 1例。转移到其他部位 2例 ,其中 1例显微全切患者术后未行放疗 ,2年后上颈髓转移死亡。结论　髓母细胞瘤恶性程度高 ,手术全切肿瘤后辅以足够剂量的放疗为主等综合治疗及定期随访可有效降低复发率和转移率 ,延长生存时间

Microsurgical treatment of medulloblasoma

Objective To discuss clinical feature and treatment effect as well as the factor affecting prognosis.Methods 20 cases with medullobalasoma were analyzed operatively and pathologically in the clinics,diagnosis,comprehensve treatment and the factor affecting prognosis,especially the relation of surgical treatment and prognosis.Results 17 patients underwent total resection and 3 subtotal resection.There were no operative mortality.19 patients were given postoperative treatment of radiotherapy,as well as individual chemotherapy.18 patients were followed up with good clinical effect.After two years 1 patient recurred in the original location,2 patients expanded to other locations,and 1 patient without postoperative radiotherapy died of the metastasis to upper cervical spinal cord.Conclusion The results indicate that medulloblasoma is a malignant tumor.Total resection and adjuvant radiotherapy and cyclically following up can effectively reduce the rate of recurring and mestastasis,thus prolonging patient's life.
