超时限腘动脉损伤诊疗分析

目的总结超时限腘动脉损伤诊治经验。方法对12例超时限腘动脉损伤患者的诊疗方法及诊疗效果进行回顾性分析。结果腘动脉损伤依临床体征作出的初步诊断准确率达92.3%。电刺激肌组织有收缩反应者保肢成功率为88.9%,术后1 a的优良率为75%。结论腘动脉损伤后超过最佳救治时间的情况较为多见。为节约时间,依临床表现作出初步诊断后即应进行急诊切开探查术。术中电刺激肌组织有无收缩反应可作为保肢与截肢的依据。保肢手术的原则是快速简单固定、动脉尽快通血、常规四筋膜间室减压、有静脉损伤者同期修复。
