胼胝体前部胶质瘤的临床特点和显微手术治疗

目的 探讨胼胝体前部胶质瘤的临床特点、应用显微手术切除的可行性及其预后。方法 对我院1998年1月至2002年6月收治的20例经手术及病理证实的胼胝体前部胶质瘤患者进行回顾性分析。结果 手术全切除12例，次全切除6例，活检2例。术后无1例死亡，3例术后出现短暂性失语和反应迟钝，2例术后癫痫小发作，1例术后出现短暂性右上肢无力，14例术后恢复良好。10例随访6个月至4年，平均生存期70周。结论 胼胝体受侵是一种对胶质瘤患者预后不利的因素。对于大部分病例，全切或次全切除肿瘤取得良好的临床效果是完全可能的。经胼胝体前部入路是治疗该区肿瘤的一个安全可行的选择。

Clinical characteristics and microsurgical treatment of anterior callosal tumors

Objective To investigate the clinical characteristics and the feasibility of surgical treatment of anterior callosal glioma by microneurosurgery technique and to evaluate the patients'prognosis.Methods 20 patients with anterior callosal glioma were operated and analyzed retrospectively in our hospital from January 1998 to June 2002.Results 12 patients underwent total ectomy,6 patients for subtotal ectomy and 2 patients for biopsy,without operative death case.The patient's postoperative recovery was well in 14 of 20 cases,but there were transient aphasia and reactive dullness in 3 cases,minor epilepsy in 2 cases and right upper extremity asthenia in 1 case.The mean survival time was 70 weeks in 10 cases of follow up.Conclusion The corpus callosum involvement is a poor prognostic factor for patients with glioma,in most cases it is possible to remove these tumors totally or sub totally with good clinical effect,and anterior transcallosal approach is a safe,feasible alternative in the management of the tumors in these regions.
