| 摘 要： | 目的观察磁共振成像（MRI）中胰腺癌各项征象及其组合对胰腺癌诊断的价值。方法选择因胰腺及其周围疾病手术患者，分为胰腺癌组及非胰腺癌组各40例。以选定MRI征象及其不同组合为参考标准，图像诊断与手术及病理检查报告对比，画出ROC曲线，观察其敏感性、特异性及准确性。结果应用动态对比增强的动脉期诊断胰腺癌效果较好。其敏感性、特异性和正确性分别为90％、85％和87．5％。应用MRCP检查，以胰管非穿透性扩张为指标，特异性达95％，敏感性52．5％。结论动态对比增强检查在应用MRI诊断胰腺癌时非常重要。MRCP对胰头癌与肿块型胰腺炎的鉴别很有必要。 |

| Abstract: | Objective To investigate the value of variable signs of MRI in diagnosing pancreatic cancer. Methods 80 patients including 40 with and without pancreatic tumors and underwent MR imaging （unenhanced and contrast material-enhanced MR）, MRCP and MR images. The diagnosis results were compared with surgery and biopsy. Results The rates of sensitivity, specificity and correctness of MRI on pancreatic tumors were 90%, 85% and 87.5%, respectively. The rates of sensitivity and specificity were 52.5 % and 95%, respectively, according to the sign of dilated pancreatic duct. Conclusion Contrast-enhanced MR imaging offers potential as a noninvasive tool for assessment of patients suspected of pancreatic tumors. MRCP is necessary for distinguishing an inflammatory pancreatic mass from a conventional pancreatic carcinoma. |
