下鼻甲黏膜下部分切除和鼻中隔矫正术治疗常年性变应性鼻炎

目的探讨下鼻甲黏膜下部分切除术和鼻中隔矫正术治疗常年性变应性鼻炎的疗效。方法采用下鼻甲黏膜下部分切除术和鼻中隔矫正术对确诊为常年性变应性鼻炎的32例患者进行手术，根据喷嚏、流涕、鼻塞和鼻痒4项指标进行术前和术后0．5、1、2年疗效评分。结果术后0．5、1年喷嚏症状评分分别为0．586±0．682和1．200±0．961．较术前1．875±0．793显著改善（P〈0．01）；术后0．5、1、2年流涕症状评分分别为0．690±0．737、1．367±0．809、1．550±0．945．较术前2．312±0．780显著改善（P〈0．01）；术后0．5、1、2年鼻塞症状评分分别为0．448±0．572、0．533±0．629、0．800±0．768，较术前2．344±0．701显著改善（P〈0．01）；术后0．5年鼻痒症状评分0．862±0．743较术前1．844±0．767显著改善（P〈0．01）。术后0．5、1年总有效率100％，术后2年总有效率90％。结论采用下鼻甲黏膜下部分切除术和鼻中隔矫正术治疗常年性变应性鼻炎近期内可有效改善患者的鼻塞、流涕。

Effect of Submucous Turbinectomy and Septoplasty on the Treatment of Perennial Allergic Rhinitis

Objective To evaluate the effect of the submucous turbinectomy and septoplasty on the treatment of perennial allergic rhinitis （PAR）. Methods In 32 PAR cases undergone submucous turbinectomy and septoplasty, their four symptoms on sneezing, rhinorrhea, nasal stuffiness and itching were evaluated according to Lanzhou criteria before and after operation. Results The symptoms improvement： the postoperative score of sneezing were 0.586 ± 0.682, 1.200 ± 0.961 after half one year, one year respectively, and shown better performance than the preoperative score 1.875±0.793 （P〈0.01）; the postoperative score of rhinorrhea were 0.690 ± 0.737, 1.367 ± 0.809, 1.550 ± 0.945 after half one year, one year, two year respectively, and shown also better performance than the preoperative score 2.312 ± 0.780 （P 〈 0.01） the postoperative score of nasal stuffiness were 0.448 ± 0.572, 0.533 ± 0.629, 0.800 ± 0.768 after haft one year, one year, two year respectively, and shown similarly better performance than the preoperative score 2.344 ± 0.701 （P 〈 0.01 ） ; the postoperative score of nasal itching was 0.862 ± 0.743 after half one year, and only shown better performance than the preoperative score 1.844 ± 0.767 （P〈 0.01 ）. Effectiveness of operation： the total effective rate was 100% after operation half one year, one year, and reached 90% after operation two year. Conclusions The result demonstrates that the submucous turbinectomy and septoplasty could improve the symptoms on nasal stuffiness and rhinorrhea in PAR.
