妊高征胎盘及脐带中细胞间粘附分子-1的表达

摘    要:目的:探讨细胞间粘附分子—1(ICAM-1)在妊高征胎盘及脐带中的表达。方法:选择20例正常妊娠及24例中、重度妊高征孕妇,以选择性剖宫产取其胎盘及脐带,用免疫组织化学的方法检测其中ICAM—1的表达。结果:妊高征胎盘合体滋养细胞染色比正常妊娠明显增强(P<0.05);胎盘的血管内皮及脐带中染色与正常妊娠相似(P>0.05)。结论:妊高征胎盘合体滋养细胞ICAM—1表达增强提示ICAM—1介导的免疫机制在妊高征的发病中可能有重要作用,而妊高征与正常妊娠绒毛血管及脐带中表达无差异表示妊高征血管内皮的激活仅局限于母体,胎儿循环不受影响。

Abstract:Objective To study the expression of intercellular adhesion molecule-I (ICAM-1) in placenta and umbilical cord in pregnancy-induced hypertension syndrome (PIH). Methods Placenta and umbilical cord were obtained from 24 cases of severe PIH and 20 normal pregnancies undergoing selected caesarean section before onset of labor. The expression of ICAM-1 in these tissues was detected by immunohistochemistry. Results The synciotrophoblast staining for ICAM-1 in placenta from PIH was significantly stronger than that from normal pregnancy (P<0.05). There was no difference between the two groups in placental endothelial and umbilical endothelial staining (P>0.05). Conclusion ICAM-1 stronger staining in synciotrophoblast in PIH than in normal pregnancy suggested that immune mechanism mediated by ICAM-1 may play an important role in the psthogenesis of PIH. That villous and umbilical endothelial staining in consistent in both groups suggests that endothelial activation is confined to matenal circulation and fetal circulation may not be affected.
