应用微板速率法替代生化分析仪检测献血者丙氨酸氨基转移酶研究

摘    要:目的应用丙酮酸氧化酶微板法、紫外-乳酸脱氢酶微板法测定丙氨酸氨基转移酶(ALT),探索适用于中小型血站及医院中心血库筛查献血者ALT的方法,以降低院内输血感染疾病的风险.方法选取体检和化验合格的381例健康献血者血标本及89例ALT异常的献血者血标本,应用全自动生化分析仪、丙酮酸氧化酶微板法、紫外-乳酸脱氢酶微板法分别测定上述标本ALT浓度.结果丙酮酸氧化酶微板法、紫外-乳酸脱氢酶微板法检测381例血清标本ALT浓度,与全自动生化分析仪结果的相关系数分别为r1=0.9935,r2=0.9862,使用3种方法检测381例血清标本ALT浓度为9.3~192 IU/L,3种方法检测结果之间差异无显著性(P>0.05);3种方法测定89例ALT异常的献血者标本,异常结果差异无显著性;紫外-乳酸脱氢酶和丙酮酸氧化酶微板法的板内变异分别为3.32%、3.54%,板间变异分别为4.89%、5.48%.结论丙酮酸氧化酶微板法、紫外-乳酸脱氢酶微板法可有效替代全自动生化分析仪测定ALT,减少因输血造成的医院感染.

Abstract:OBJECTIVE To apply the two methods for assaying concentration of alanine transaminase (ALT) in (order) to reduce the risk of blood transfusion infection. METHODS Using plate kinetic method to determine ALT levels of 381 blood samples from the healthy blood donors and 89 blood samples collected from the (refused) (donors) with ALT abnormal. The (results) were compared with the values recommended by IFCC. RESULTS The ALT (concentration) of 381 healthy donors were ranging from 9.3 IU/L to 192 IU/L. Compared with the ALT (values) tested by automatic biochemical analyzer, the value tested by microtiter plate kinetic method had no (significant) difference ((P>0.05)) and with a correlation value of 0.9935 and 0.9862, respectively. The results of 89 ALT positive samples determined by the three methods also had no significant difference. CONCLUSIONS The microtiter plate (kinetic) method can effectively displace automatic biochemical analyzer for blood donor screening, which can efficiently reduce the risk of blood transfusion infection.
