摘    要:目的 探讨男性性激素水平(雌二醇、睾酮、孕酮)与卒中后焦虑、抑郁症的关系.方法 检测和比较30例男性卒中后焦虑、抑郁症患者和30例非卒中后焦虑、抑郁症患者的血清雌二醇、睾酮、孕酮的水平,并应用汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)对卒中后焦虑、抑郁症患者进行临床评分,同时作相关分析.结果 男性卒中后焦虑、抑郁症患者血清雌二醇水平均显著低于对照组(P<0.05),雌二醇水平与HAMA分和HAMD分呈负相关(r=-0.540,P<0.05;r=-0.503,P<0.05).睾酮水平显著低于对照组(P<0.05),睾酮水平与HAMA分和HAMD分呈负相关(r=-0.535,P<0.05;r=-0.602,P<0.05).两组的孕酮水平无显著变化(P>0.05).结论 卒中后焦虑、抑郁症存在明显的性激素水平的紊乱,与脑卒中的发病发展有着密切的关系.血清雌二醇、睾酮可能成为卒中后焦虑、抑郁症的生物学标记.

Abstract:Objective To explore the relationship between male sexual hormone (estradiol, testosterone, progesterone) and post--stroke anxiety or post--stroke depression. Methods The serum estradiol, testosterone, progesterone levels were measured in 30 post--stroke anxiety or post--stroke depres- sive patients and 30 post--stroke patients without anxiety or depression. The degree of anxiety and de pression was clinically assessed with Hamilton Anxiety Scale(HAMA) and Hamilton Rating Scale for depression(HAMD). Correlation analysis was conducted. Results The estradiol level of male post-- stroke anxiety or post-- stroke depressive patients was significantly lower than that of controls (P 0.05), and showed negative correlation with the scores of HAMA (r= 0. 540,P 〈 0.05) and HAMD (r =--0. 503, P〈 0.05 ). The testosterone level of male post--stroke anxiety and male post--stoke de- pressive patients was significantly lower than that of controls (P ~ 0.05), and showed negative correla- tion with the scores of HAMA (r =--0. 535,P~0.05) and HAMD (r=-0. 602,P〈0.05). The levels of progesterone between two groups were not significantly different (P〉 0.05). Conclusions The sexu- al hormone disturbance was found significantly in post--stroke anxiety and post--stroke depressive patients, which has close relationship with morbidity and development of stroke. The measurement of estradi- ol and testosterone level may be used as a biomarker for post--stroke anxiety and post--stroke depression.
