速度向量成像技术评价终末期肾病患者颈总动脉管壁弹性

摘    要:目的:探讨速度向量成像技术(VVI技术)对终末期肾病(ESRD)患者颈总动脉管壁弹性评估的临床意义。方法:对实验组36例需要血液透析的ESRD患者进行高频超声检查,应用VVI技术分别测量颈总动脉管壁6个节段(前壁、后壁、前外侧壁、前内侧壁、后外侧壁及后内侧壁)的径向最大运动速度、切向最大应变及切向最大应变率,并进行分析。对照组34例排除冠心病、糖尿病、高血压、高血脂、肥胖症、肾移植及吸烟酗酒等病史。结果:①实验组与对照组间6个节段间的最大应变(Smax)、最大应变率(SRmax)的差异无统计学意义(P〉0.05),而两组间的最大速度(Vmax)差异具有统计学意义(P〈0.05),前壁、前外侧壁及后外侧壁大于后壁、前内侧壁及后内侧壁;②两组对应各节段间的Vmax、Smax及SRmax的差异有统计学意义(P〈0.05)。结论:VVI技术可检测ESRD患者颈总动脉管壁弹性的变化。颈总动脉运动的机械不一致性和不同节段的搏动指标有差别,这一结论对ESRD患者颈总动脉管壁弹性的评价有重要临床意义,并且有可能作为其检测指标。

Abstract:Objective:To investigate the clinical value of velocity vector imaging in assessing the elastic change of common carotid artery in patients with end-stage renal disease.Methods:36 patients who needed haemodialysis with end-stage renal disease belonged to experimental group.34 persons without coronary heart disease,diabetes,hypertension,hyperlipoidemia,adiposity,renal transplantation,smoking and dipsopathy were supplied as normal control group. Maximum of velocity(Vmax),maximum of strain(Smax) and maximum of strain rate(SRmax) of 6 segments(anterior wall,anterolateral wall,anteromedial wall,posteromedial wall,posterior wall,posterolateral wall) were measured,which were supplied by VVI automatic analysis software.Results:Smax and SRmax had no significant differences in any segment in both experimental group and normal control group(P〈0.05).But Vmax of every segment had significant difference in the two groups(P〉0.05).Vmax of anterior wall,anterolateral wall and posterolateral wall were higher than those of posterior wall,posteromedial wall and anteromedial wall.Vmax,Smax and SRmax of corresponding segments had significant differences in the two groups(P〈0.05).Conclusion:The change of common carotid elasticity in patients with end-stage renal disease can be detected by VVI.The machine inconformity and the pulsation of each segment in the common carotid artery have difference.The conclusion is important to evaluate the elasticity of common carotid artery in patients with end-stage renal disease.
