斜T形钢板与外固定架均结合克氏针内固定治疗桡骨远端C型骨折的疗效对比

目的比较斜T形钢板与外固定架均结合克氏针内固定治疗桡骨远端C型骨折的疗效对比分析。方法回顾性分析自2006—01—2013-06分别采用斜T形钢板内固定与外固定架结合克氏针内固定治疗桡骨远端C型骨折44例。结果所有患者获得12～18个月（平均16．5个月）的随访。术后12个月采用腕关节功能Gartland—Werley评分对术后疗效进行评估：内外固定组优良率分别为81．0％（17，21）、82．6％（19／23），并发症发生率分别为9．52％（2／21）、13．04％（3／23）。2组比较差异无统计学意义（P〉0．05）。术后6个月内固定组掌倾角、尺偏角明显优于外固定组，差异有统计学意义（P〈0．05）。结论2种固定方式治疗桡骨远端C型骨折均可达到较为满意的临床效果，斜T形钢板内固定术后短期内影像学结果稍好，对于远期疗效而言，选用外固定架结合克氏针内固定有创伤小、恢复快、成本低、操作方便等特点。

Oblique T-plate fixation vs. external fixator combined with Kirschner wire for treatment of type C distal radius fractures

Objective To compare and analyze the efficacy of oblique T-plate fixation and external fixation with limited open local Kirschner wire fixation for treatment of type C distal radial fractures. Methods From January 2006 to June 2013, 44 cases of type C distal radius fractures were treated by the oblique T-plate fixation and external fixation with limited open local Kirschner respectively. Results All patients were followed for 12 to 18 months after operation（average 16.5 months）. Twelve months after operation, according to Gartland-Werley evaluation of distal radius fracture postoperative efficacy criteria： the internal fixation and external fixation group, the excellent and good rate was 81.0%（17/21 ）, 82.6%（19/23）; The complication rate was 9.52%（2/21）, 13.04%（3/23）, the difference was not statistically significant （P 〉0.05）. Six months after internal fixation group palmar angle ulnar deviation were significantly better than the external fixation group. The difference was statistically significant （P 〈0.05）. Conclusion Oblique T plate fixation of group has better imaging outcomes with a reliable fixation,but both of the groups can achieve satisfactory clinical results.For long-term efficacy, optional external fixation with Kirschner wire fixation hasadvantages such as less trauma, quicker recovery, lower cost, easier to operate.
