血清CEA、CA125、NSE与CYFRA21-1联合检测在肺癌诊断中的应用

目的探讨肿瘤标志物癌胚抗原（CEA）、糖类抗原125（CA125）、神经元特异性烯醇化酶（NSE）、细胞角蛋白21-1片段（CYFRA21-1）检测对肺癌诊断的临床意义。方法采集40例健康体检组，56例良性疾病组及肺癌组92例，用罗氏2010电化学发光检测其血清中CEA、CA125、NSE、CYFRA21-1的含量。结果肺癌患者的4种血清肿瘤标志物水平均明显高于肺部良性疾病组及健康体检组，有显著性差异（P〈0.01），CYFRA21-1在鳞癌中阳性率最高，明显高于其他类型的肺癌，CA125、CEA在腺癌中阳性率最高，NSE则在小细胞癌最为明显。4者联合检测可提高阳性率至95.6%，显著高于各单项的敏感性。结论单项检测特异性虽高，但灵敏度较低，联合检测可大大提高灵敏度和特异性，具有较高的实用价值。

Clinical Application of Tumor Marker CEA、CA125、NSE、CYFRA21-1 in Diagnosis of Lung Cancer

Objective To study the clinical of serum CEA, CA125, NSE and CYFRA21 - 1 in the diagnosis of lung cancer. Methods Collect 40 health persons,56 patients of pulmonary benign lesion, 92 patients of pulmonary carcinoma, and detect the contents of CEA, CA125, CYFRA21 - 1 and NSE in the serum by electrochemiluminescence immunoassay. Results The level of the four serum tumor markers in lung cancer patients was significantly higher than those in pulmonary benign lesion and health persons （ P 〈 0.01）. The level of CYFRA21 - 1 in patients with squamous- cell carcinoma was significandy higher than that in other subtypes. The level of CA125 and CEA in patients with glandular carcinoma was significantly higher than that in other subtypes . The level of NSE in patients with small - cell lung cancer was signifieandy higher than those in the other subtypes of lung cancer. The positive of combining test is 95.6%, that was obviously higher than only testing the single item. Conclusion The sensitivity and specificity of the four combined tumor markers was obviously higher than single tumor marker, there was price in the diagnosis of lung cancer.
