超声造影鉴别诊断子宫内膜增生与早期子宫内膜癌

摘    要:目的 探讨CEUS鉴别诊断子宫内膜增生与早期子宫内膜癌的价值。方法 对32例I期子宫内膜癌患者(子宫内膜癌组)、29例子宫内膜增生患者(子宫内膜增生组)及30名健康志愿者(正常对照组)进行子宫内膜CEUS检查,用QLab定量分析软件进行定量分析,获取时间-强度曲线(TIC)及相关灌注参数到达时间(AT)、达峰时间(TTP)、上升时间(RT)、基础强度(BI)、峰值强度(PI)和强度相对值(EI)。结果 与正常对照组比较,子宫内膜增生组所有参数差异均无统计学意义(P均>0.05),子宫内膜癌组PI、EI、AT、TTP、RT差异有统计学意义(P均<0.05),BI差异无统计学意义(P>0.05)。结论 CEUS对于定性诊断子宫内膜增生与早期子宫内膜癌具有一定临床价值。

Abstract:Objective To explore the clinical value of CEUS in differential diagnosis of endometrial hyperplasia and early endometrial carcinoma. Methods Totally 32 patients with stage I endometrial carcinoma (endometrial carcinoma group) and 29 patients with endometrial hyperplasia (endometrial hyperplasia group), as well as 30 volunteers (normal control group) underwent CEUS. QLab quantitative analysis software was used for quantitative analysis, time-intensity curve (TIC) and perfusion parameters including arrival time (AT), time to peak (TTP), rise time (RT), basic intensity(BI), peak intensity (PI) and relative rise in intensity (EI) were acquired and compared. Results All parameters of endometrial hyperplasia group were not statistically different with those of normal control group (all P>0.05). PI, EI, AT, TTP and RT of endometrial carcinoma group were statistically different with those of normal control group (P<0.05), but BI had no difference between endometrial carcinoma group and control group. Conclusion CEUS is helpful to the differential diagnosis of endometrial hyperplasia and early endometrial carcinoma.
