摘    要:目的 应用实时剪切波弹性成像(SWE)技术定量评估不同时期下肢深静脉血栓(DVT)弹性的变化。方法 选取71例经常规超声诊断为下肢DVT的患者,根据发病时间和超声图像特点分为急性期组、亚急性期组和慢性期组;应用SWE超声诊断仪测量各组患者下肢DVT的平均杨氏模量值,并进行统计学分析。结果 急性期、亚急性期、慢性期组平均杨氏模量值分别为(7.65±1.69)kPa、(14.41±2.36)kPa和(26.16±4.07)kPa,3组间差异有统计学意义(F=267.558,P<0.01)。结论 SWE技术可定量不同时期下肢深静脉血栓的平均杨氏模量值范围,为选择治疗方案和评估肺栓塞风险提供重要依据。

Abstract:Objective To explore the difference of Young's modulus in deep vein thrombosis (DVT) of lower extremity at different phases with real-time shear wave elastography (SWE). Methods A total of 71 patients diagnosed with DVT of lower extremity by conventional ultrasound were enrolled and divided into 3 groups (acute group, sub-acute group and chronic group) according to the onset of symptoms and ultrasonic features. The Young's modulus of DVT were obtained with real-time SWE and analyzed with statistical methods. Results Young's modulus of acute group, sub-acute group, and chronic group was (7.65±1.69)kPa, (14.41±2.36)kPa and (26.16±4.07)kPa, respectively, and the differences were statistical significant (F=267.558, P<0.01). Conclusion SWE technique can quantitatively measure the hardness of DVT and the Young's modulus in difference periods of lower extremity, therefore providing important evidences for treatment planning and evaluating the risk of pulmonary embolism.
