用McAb-AST对黑热病患者疗效考核的研究

用单克隆抗体-抗原斑点试验(McAb-AST)又一次对30例黑热病患者作血清循环抗原检测,以研究对疗效考核的价值,其中5例作了治疗前和治疗后的比较,血清循环抗原转为阴性反应者一例,由强阳性转为中度阳性及持续呈阳性者各2例,均与患者临床表现及原虫查获情况吻合。另25例经锑剂一疗程后一年左右用本法测检,13例呈阴性,12例呈中度或弱阳性反应。251例正常人及结核等传染病患者均呈阴性,无假阳性出现。说明本法特异、敏感,并进一步证实了对黑热病的疗效考核有价值,且方法简易可行,易为病人接受。提示可望取代骨髓涂片检查。

DETECTION OF CIRCULATING ANTIGENS BY McAb-AST FOR EVALUATION OF THERAPEUTIC EFFECT OF KALA-AZAR CASES

It has recently been reported by theauthors that circulating antigens of Leish-mania donovani are detectable with McAb-AST in the sera from Kala-azar patients.In the present study, the post therapeuticmeasurements of 30 Kala-azar cases by thesame method are presented. Five of thecases were studied before and after thetreatment for comparison. The positive re-action of circulating antigens of the serashowed a corresponding relationship to thepresence of protozoan in the bone marrowsmear or to the clinical symptoms, i. e.the Leishmania donovani amastigotes wererevealed in the bone marrow smears of 2patients whose sera showed strong positivereactions in McAb-AST although they hadbeen repeatedly treated before. Serumfrom a clinically cured patient which hadbeen positive before the treatment wasconfirmed by the negative circulating an-tigens reaction. Moreover, the sera from25 patients about 1 year after a stibii hex-onas treatment were measured and 13 ofthem showed negative, the others showedmoderate or weak positive. All of 251 serafrom normal donors including patients withtuberculosis or other infective diseases werenegative in circulating antigens reactionsand no false positive reactions were ob-served. Because of its specificity, sensitivityand simplicity, McAb-AST technique canbe a new valuable method for the evalua-tion of therapeutic effect of Kala-azar casesand can easily be accepted by the patients.
