丹参预防椎板切除术后硬膜外粘连的实验研究

摘    要:为观察丹参水溶性成分、游离脂肪块、明胶海绵预防硬膜外粘连的效果。将32只南昌大白兔随机分为4组:空白组(C)、明胶海绵组(GS)、游离脂肪组(FF)及丹参组(RA),行L5、L7全椎板切除术,于术后2周、4周、6周、8周分别大体观察、组织学观察及透射电镜观察。结果显示FF组和C组,FF组和GS组,RA组和C组,RA组和GS组之间分别相比均差别有显著性意义(P<0.05);FF组和RA组,C组和GS组之间相比差别无显著性意义(P>0.05)。半流质状丹参水溶性药液具有抗纤维化、抑制成纤维细胞和炎性细胞的渗出、激活单核/吞噬细胞系统、促进胶原纤维蛋白降解的作用,可达到很好的三维防粘效果。FF组的生物相容性优于RA组,预防粘连的作用较好。明胶海绵在体内存留时间短(4周),晚期防粘效果与C组则无明显差异。

Abstract:To observe the effect of the Radix Salviae Miltiorrhizae's water-soluble ingredients, free fat and gelatin sponge on prevention of the epidural fibroscar adhesion (EPFA). The total laminectomies were performed at L 5 and L 7 in thirty two NCW rabbits, which were divided into four groups randomly. These groups were control group (C), free fat group (FF), gelatin sponge group (GS) and Radix group (RA). Gross observation, histological examination, and transmission electron microscope were respectively undertaken at 2, 4, 6 and 8 weeks after operation. The results showed that there were the significant differences between FF and C, FF and GS, RA and C, RA and GS (P<0.05), while there were no significant differences between FF and RA, C and GS (P>0.05).The semi liquid water soluble Radix can prevent the migration of fibroblasts and inflammatory cells, inhibit the proliferation of fibroblasts, stimulate the mononuclear phagocyte system, and promote the degradation of the collagen fibrils. The free fat had a better histocompatibility and ability of reducing the fibroscar than the Radix. It had a good effect in preventing the fibrosis form the posterior. The gelatin sponge was degraded and absorbed in early period (4 weeks), but in late period there was no significant difference between the C and FF groups.
