基于ICD-9编码的ISS创伤评分方法研究

目的　探讨一种新的以ICD 9编码为基础的ISS创伤评分方法的可行性及其实用价值。方法以我室编制的ICD 9编码与AIS分值对应表为基础 ,将创伤患者的ICD 9编码转换为AIS分值 ,通过两种方法 ,计算ISS分值。运用logistic回归分析及受者工作特征曲线 (ROC)分析方法评价所得ISS分值对患者结局的预测价值。结果　两种方法计算所得ISS分值对患者结局的影响均有统计学意义 (P <0 .0 5 ) ,ROC曲线下面积达到 0 .85以上。结论　基于ICD 9编码的ISS创伤评分方法所得分值对患者结局具有很好的预测效果 ,此创伤评分方法有很高的实用价值

Exploration of a newly developed trauma severity scaling method of ISS based on ICD-9 codes

Objective To explore the feasibility and utility of a newly developed trauma severity scaling method of ISS method based on ICD 9 codes.Methods With the corresponding table of ICD 9 codes of trauma diseases and AIS values developed by us,the ICD 9 codes of diseases of trauma patients were converted to AIS values. The derived AIS values were then used to work out ISS values by two different ways. Logistic regression and ROC analysis were used to evaluate the effect of the ISS values in predicting the outcomes of trauma patients.Results The ISS values derived by two ways have statistically significant effect on the outcomes of trauma patients (P<0.05) and the areas under the ROC curves are all larger than 0.85.Conclusion The trauma severity scaling method of ISS based on ICD 9 codes is effective on prediction of outcomes of trauma patients and has high practical utility.
