慢性肾功能衰竭的彩色多普勒能量图分析

目的：探讨慢性肾功能衰竭患者肾脏的彩色多普勒超能量图（CDPI）表现及临床意义。方法：采用二维超声显像和CDPI技术对115例慢性肾功能衰竭患者和45例健康成人的肾脏进行检查，对比分析其肾脏体积、皮质厚度、血流动力学变化及血流灌注情况。结果：慢性肾功能衰竭代偿期，肾脏体积无缩小，皮质厚度正常，CDPI显示肾脏血流丰富，呈连续的树枝样分布，主肾动脉血流参数与对照组相差不显著（P＞0．05）；慢性肾功能衰竭失代偿期，肾脏体积缩小，皮质变薄，CDPI显示肾内血流减少，主肾动脉血流速减低，以舒张期为明显，收缩期、舒张期峰值距差增大，阻力指数增高，与对照组相差显著（P＜0．001）。慢性肾功能衰竭期，肾脏体积明显缩小，皮质变薄，CDPI显示肾内血流进一步减少，主肾动脉血流速更低，阻力指数增高，与对照组相差显著（P＜0．001）。结论慢性肾功能衰竭患者的肾脏CDPI表现，反映了肾脏的血流灌注情况，对慢性肾功能的诊断、分期、治疗监测和预后评估都具有重要的意义。

Evaluation of Color Doppler Power Imaging in the Diagnosis of Chronic Renal Failure

Objective:To explore the value of color Doppler power imaging(CDPI)in the diagnosis of chronic renal failure(CRF).Methods：One Hundred and fifteen cases with CRF and 45 healthy persons were examined by two-dimensional sonography and CDPI.Results:In the decompensatory phase of CRF,the decreases including volume and thickness of the kidney cortex were significantly different as compared with those of the control group(P＜0.001).The blood flow signals of their main renal arteries and branches showed a tendency to begradually weakened and disappeared.The velocity of main renal artery was lowered,while the resistant index was higher than those of the control group(P＜0.001).Conclusions:It is shown that CDPI is useful for the diagnosing,monitoring treatment and estimating the prognosis of CRF.
