Tei指数评价房间隔缺损患者手术前后右心室功能的临床研究

目的应用Tei指数评价房间隔缺损(ASD)患者手术前后右心室功能的临床价值。方法应用超声多普勒技术对33例房间隔缺损患者手术前及手术后1周、1个月、3个月、6个月的右室射血分数(RVEF)、三尖瓣E峰与A峰峰值速度、右室等容收缩时间(ICT)与等容舒张时间(IRT)、右室射血时间(ET)、肺动脉收缩压(PASP)及Tei指数进行测定。结果在各时间点Tei指数逐渐减小,差异均具有显著性意义(P<0.05),其余各指标各时间点间也存在不同程度的差异。结论ASD患者右心功能确有减退,Tei指数是综合检测右室收缩和舒张功能的可靠指标,手术修补ASD可以使心功能得到改善并防止心功能持续恶化。

Tei Index Evaluation of Right Ventricular Function in Patient with Atrial Septal Defect and After Operation

Objective Tei index assesses the clinical value of right ventricular function before and after operation of patient with atrial septal defect (ASD).Methods Thirty-three patients with atrial septal defect,who had accepted operation,were included in this evaluation.Right ventricular ejection fraction (RVEF),tricuspidal annular E peak velocity and A peak velocity,right ventricular isovlumetric contraction time (ICT) and isovolumetric reaxation time (IRT),right ventricular ejection time (ET),pulmonary artery systolic pressure (PASP) and Tei index were measured by echocrdiography l week,1 month,3 months and 6 months before and after the operation.Results Tei index decreased gradually as time going after operation and the differences were significant between different time points(P<0.05).There were statistical differences in all other measurements but not all the parameters were different.Conclusions The right venatricular function in patient with ASD was impaired indeed.Tei index is accurate and simple in estimateing right ventricular systolic and diastolic function.Surgical treatment could improve right ventricular function and prevent it from further deterioation.
