小细胞肺癌的CT表现与肿瘤标志物 NSE、CYF21-1、CEA 的相关性研究

摘    要:目的探讨小细胞肺癌的CT表现与血清肿瘤标志物NSE、CYF21-1、CEA浓度的相关性。方法收集整理经病理证实的21例小细胞肺癌患者的CT资料与血清肿瘤标志物NSE、CYF21-1、CEA的浓度,将CT表现分型间肿瘤标志物进行对比分析。结果密度均匀与密度不均(15/6)、有心包积液与无心包积液(3/18)者的血清CEA浓度比较均有统计学差异(P0.05);有血管包埋与无血管包埋(14/7)、有胸腔积液与无胸腔积液(10/11)、有胸膜转移与无胸膜转移(9/12)、有胸外转移与无胸外转移(8/13)患者NSE浓度比较,差异均有统计学意义(P0.05);轻度强化与中度强化(13/8)、有胸外转移与无胸外转移(8/13)患者CYF21-1浓度比较,差异均有统计学意义(P0.05)。结论 SCLC的密度均匀与否与血清CEA浓度有一定相关性,强化程度与血清CYF2-11浓度有一定相关性,血管受累及胸膜转移与血清NSE浓度有一定相关性,胸外转移与NSE、CYF2-11血清浓度有一定相关性,胸腔积液和心包积液分别与血清NSE、CEA浓度有一定相关性。

Correlation between CT findings and tumor markers NSE, CYF21-1 and CEA in patients with small cell lung cancer
Abstract:Objective To investigate the correlation between the CT findings of small cell lung cancer and serum tumor markers NSE, CYF21-1 and CEA. Methods 21 cases of small cell lung cancer confirmed by pathology were collected for CT data and serum concentration of tumor markers NSE, CYF21-1 and CEA. The tumor markers of different types of CT manifestations were compared. Results There was a significant difference in serum CEA concentration between uniform density and uneven density types(15 cases/6 cases), and also between those with and without pericardial effusion (3/18)(P<0.05). Patients with and without vascular embedding(14/7), with and without pleural effusion(10/11), with and without pleural metastasis(9/12), with and without extrathoracic metastasis(8/13) all showed significant difference in NSE concentration(P<0.05). The levels of CYF21-1 were statistically different between mild and moderate enhancement (13/8) and between patients with and without extrathoracic metastasis (8/13)(P<0.05). Conclusion SCLC density is correlated with serum CEA concentration. The degree of enhancement is correlated with serum CYF2-11 concentration.Vessels invasion and pleural metastasis were correlated with serum NSE concentration. Extrathoracic metastasis showed correlation with serum NSE and CYF2-11 concentration. Pleural effusion and pericardial effusion were correlated with serum NSE and CEA concentration.
