扩张型心肌病左房舒缩功能的应变率研究

目的 应用应变率成像技术评价扩张型心肌病(DCM)患者左心房收缩与舒张功能.方法 对照组20 例和DCM组21 例分别测量左房侧壁(LALW)、前壁(LAAW)、下壁(LAIW)基、中、尖各节段及房顶部的收缩期应变率(Ssr)、舒张早期应变率(Esr)和舒张晚期应变率(Asr),并进行对比分析.结果 DCM组Ssr、Esr、Asr 均低于对照组;对照组收缩期各房壁尖段及房顶部峰值Ssr均高于基段,舒张期除LALW外,其余各房壁Esr、Asr变化同收缩期;DCM组收缩期和舒张期LAIW尖段及房顶部Ssr、Esr、Asr均低于基段,LALW和LAAW各节段与基段相比无明显变化;对照组和DCM组的LALW、LAAW、LAIW的 Ssr与Esr、Asr负相关.结论 对照组左心房舒缩运动的应变率指标由左房壁尖段、房顶部向中段、基段方向递减,DCM组此规律消失.

Left Atrial Systolic and Diastolic Function Evaluated by Strain Rate Imaging in Patients with Dilated Cardiomyopathy

Objective To assess left artrial(LA)systolic and diastolic function with strain rate imaging(SRI)in patients with dilated cardiomyopathy(DCM).Methods Totally 21 DCM patients and 20 controls were studied.Systolic peak strain rate(Ssr),early diastolic strain rate(Esr),late diastolic strain rate(Asr)were measured in different segments of LALW(left atrial lateral wall)and LAAW(left atrial anterior wall) and LAIW(left atrial inferior wall).Results The Ssr,Asr,Esr of LA walls in the DCM group were lower than those in control group.In control group,the apical and top Ssr were higher than the basal part of LA wall.Excluding LALW,the same result can be seen in Esr and Asr.In DCM group,the Ssr,Asr,Esr of LAIW at the apical and top were lower than the basal and middle part,while no difference was found between the middle,apical,top and basal part in LALW and LAAW.In control and DCM group,Ssr were negatively correlated with Esr and Asr of LA wall.Conclusions LA systolic and diastolic function in control group was decreased from the apical and top part to the basal and middle,while the DCM patients lost the rule.
