对高血压患者臂式电子动态血压仪测得的瞬时血压值与台式汞柱式血压计的血压测量值的比较

摘    要:目的:针对高血压患者研究采用臂式电子血压仪和台式汞柱式血压计测得的血压值是否存在差异,及其差异产生的原因。方法:连续入选在高血压专科门诊就诊和住院的高血压患者291人,在每位入试患者均在固定的时间、取坐位休息15分钟以上、避免各种环境因素的刺激以及降压药物的干扰并且在两侧脉搏触诊对称的情况下,同步采用臂式电子动态血压仪和台式汞柱式血压计进行血压的测量,观察两种测量方法测得的血压值的差异。结果:臂式电子动态血压仪测得的瞬时收缩压值较台式汞柱式血压计测得的读数高出7.0±16.5mmHg(P=0.000);臂式电子动态血压仪测得的瞬时舒张压值较台式汞柱式血压计测得的高3.8±12.3mmHg(P=0.000)。在校正了体重指数、腰围和血糖、电解质等多项因素后,多元线性回归方法显示甘油三酯和血尿素氮是血压差异的影响因素。结论:在高血压患者中,标准坐位的情况下臂式电子动态血压仪的瞬时血压测量值不管是收缩压还是舒张压均显著高于台式汞柱式血压计的测量值,其收缩压的差异更大。甘油三酯和血尿素氮是两种测量方法差异的独立影响因素。
