| 摘 要： | 目的：回顾性分析1731例经皮腰椎间盘切除术（PLD）的病例，总结PLD的中远期疗效及其主要并发症的防治。方法：对1991年2月至2001年3月在我院接受PLD治疗的1731例病人进行回顾性总结，对472例随访3－5年的病例进行了中期疗效分析；对385例随访6－9年的病例作了远期疗效分析。研讨了PLD术后腰痛加重的防治方法。结果：PLD中期有效率为86．7％，远期有效率为78．2％。术后腰痛加重与腰肌痉挛、小关节绞锁有关。结论：经皮腰椎间盘切除术是1种安全有效、康复快、并发症少的治疗腰椎间盘突出症的微创方法，值得在临床上推广应用。

| Abstract: | Objective To study the medium term and long term therapeutic effect of percutaneous lumbar discectomy (PLD), and to evaluate the prevention and treatment of its complications by retrospectively analyzing 1 731 cases who received PLD procedure Methods 1 731 PLD procedures performed from Feb 1991 to Mar 2001 were retrospectively studied The medium term therapeutic effect in 472 cases followed from 3 to 5 years and the long term effect in 385 cases followed from 6 to 9 years were analyzed The prevention and treatment of the aggravating lumbodynia after PLD were also investigated Results The medium term and long term effective rate was 86 7% and 78 2%, respectively The aggravating lumbodynia after PLD was related to psoas muscle spasm and lumbar small joint disorder Conclusion PLD is an effective, safe, and minimal invasive treatment method with less complications |
