电子阴道镜检查鉴别宫颈病变的临床价值

摘    要:目的探讨电子阴道镜检查诊断宫颈病变的临床价值。方法对6109例行电子阴道镜检查患者及同时行镜下定位取活组织病理学检查的3138例患者的临床资料进行回顾性分析,分析阴道镜检查结果与病理检查结果的关系。结果发现病变3138例(占51.37%),经病理确诊为宫颈原位癌2例(占0.064%),鳞癌3例(占0.096%),腺癌1例(占0.032%),CIN Ⅰ 167例(占5.32%),CIN Ⅱ 21例(占0.67%),CIN Ⅲ 7例(占0.22%),HPV感染51例(占1.63%),慢性宫颈炎2877例(占91.68%)。阴道镜对宫颈病变(包括湿疣、炎性病变、癌前病变和宫颈癌病变)的诊断正确性较高;异常阴道镜图像在癌前病变和宫颈癌的病例中发生率均较高。结论电子阴道镜检查对宫颈病变的早期诊断具有重要的筛查作用。

Abstract:Objective To evaluate the worthiness of electronic coploscopy in the diagnosis of cervical disease. Method To review 6109 cases of colposcopy. And 3138 cases were given pathological examination at the same time. The results of colposcopy and pathological examination were analyzed. Result 3 138 cases were detected pathological changes. After pathology diagnosis, there were 2 cases with carcinoma in situ(0.064%), 3 cases with squamous cancer(0.096%), 1 case with gland cancer(0.032%); 195 cases of CIN, included in 167 cases of CIN Ⅰ (5.32%), 21 cases of CIN Ⅱ(0.67%), 7 cases of CIN Ⅲ(0.22%), 51 cases of HPV infection(1.63%).The other 2 877 cases were chronic inflammation(91.68%).It was high validity to diagnose neck diseases(include wet wart, inflammation, preceding or cancer pathological changes) by electronic coploscopy. Abnormal picture of colposcopy were often happened in preceding or cancer pathological changes. Conclusion It had important value to screen cervical pathological changes by colposcopy. And it could be the gist of forepart diagnose.
