乳腺肿瘤患者术前抑郁焦虑情绪及影响因素

目的 探讨乳腺肿瘤患者术前抑郁和焦虑状况及其影响因素。方法 应用临床心理问卷(EPQ、HAMD、HAMA、LES、SSRS、DSQ)对术前的乳腺肿瘤患者进行评定,以HAMD、HAMA总分为因变量,其它心理社会因素为自变量,进行多元逐步回归分析。结果 乳腺肿瘤患者术前抑郁和焦虑状态的发生率分别是 36. 21%、60. 34%;HAMD总分与负性生活事件刺激量、DSQ不成熟防御因子分、EPQ的N分呈显著正相关,与社会支持总分和年龄呈显著负相关。HAMA总分与患者的经济状况、主观支持分呈显著负相关,与DSQ不成熟防御因子分、EPQ的N分呈显著正相关。结论 乳腺肿瘤患者术前存在较普遍焦虑和抑郁情绪。术前经历较多负性生活事件、采用不成熟防御方式、具有神经质人格、年龄较小、经济状况较差的乳腺肿瘤患者术前较易出现焦虑和抑郁情绪,而良好的社会支持能减轻术前情绪问题。

Depression and anxiety of patients with breast tumor and influencing factor before operation

ObjectiveTo explore depression and anxiety of patients with breast tumor and influencing factor before operation.MethodsCharacter trait, coping styles, life events, social supports, depression and anxiety were measured for patients with breast tumor by the clinical psychological rating scale(EPQ,HAMD,HAMA,LES,SSRS,DSQ)in before operation one day.ResultsThe incidence rate of depressive and anxious condition of patients with breast tumor is 36.21% and 60.34% individually. The total score of HAMD is positively associated with negative life events and immature defense factor score of DSQ and N score of EPQ, but negatively associated with total score of social support and age. The total score of HAMA is positively associated with immature defense factor score of DSQ and N score of EPQ,but negatively associated with score of subjective social support and economic condition.Conclusion There are more depressive and anxious emotion in patients with breast tumor before operation. The patients who experienced more negative life events and adapted immature defense and have poor economic condition have severe emotional question. The social supports can relief the emotional reaction of patients before operation.
