前列腺三维磁共振波谱的临床初步研究

摘    要:目的评价三维磁共振波谱成像(three dimensionalMRspectroscopicimaging ,3DMRSI)联合MRI对鉴别前列腺癌和良性前列腺增生的价值。方法对15例临床疑有前列腺癌患者行MRI和3DMRSI检查,并将3DMRSI检查结果与手术病理结果对照。结果15例患者全部顺利完成3D增长MRSI检查,其中良性前列腺增生4例、前列腺癌11例,良性前列腺增生的枸橼酸盐(citrate ,cit)峰位于2 85~3 0 3mg/L ;胆碱(choline ,cho) +肌酸(creatine ,cre)峰值位于0 9~1 67mg/L ;中央腺体的(cho +cre) /cit在0 48~0 5 3 ;前列腺癌灶区的cit峰值明显降低,位于0 5 7~0 85mg/L ;cho +cre峰值改变不明显,位于0 86~1 16mg/L ;(cho +cre) /cit比值增大,其比值为1 48~2 0 3。结论用3DMRSI检查前列腺可得到有意义的谱线,与病理对照研究的初步结果显示3DMRSI有助于鉴别列腺癌和良性前列腺增生,与MRI结合能显示肿瘤范围。

Abstract:Objective To evaluate diagnostic value of three-dimensional MR spectroscopic imaging(3D MRSI) combined with MRI on the distinguishing of prostate cancer and prostate hyperplasia. Methods 15 clinic highly suspected patients of prostate cancer were taken MRI and 3D MRSI and comparing the results with pathologic results after operation. Results 15 cases were successfully taken 3D MRSI and found that 4 cases were prostate hyperplasia, 11 cases were prostate cancer. In the results of prostate hyperplasia, the peak value of Citrate(Cit)is in 2.85~3oline (Cho) and Creatine (Cre) is in 0.9~1.67 mg/L; the ratio of (cho+cre)/cit in center gland is 0.48~0.53; the peak value of Cit is obviously dropped to 0.57~0.85 mg/L; the peak value of Cho+Cre change little in 0.86~1.16 mg/L; But the ratio of (cho+cre)/cit is increased to 1.48~2.03. Conclusion 3D MRSI can get significative spectrum and is useful to distinguish prostate cancer and prostate hyperplasia with the primary results of pathologic comparison. It also can display the area of cancer combined with MRI.
