刘宏第熄火化瘀补虚分期治疗紫癜性肾炎

刘宏第教授认为紫癜性肾炎可归属“血证”范畴，具有反复发作，难以速愈特点。系先天禀赋不足，复为外邪触动所致。可分为急性期与迁延期。急性期多因内有阳明积热，或阴虚燥热或小儿稚阴之体，复为风热湿毒等外邪侵袭，导致血热，迫血妄行，故此期多为实热证，病位在肺胃，治疗当以祛邪为主；病程日久，则进入迁延期，热盛伤阴，阴伤及气，可见气虚、阴虚火旺、气阴两虚，病位在脾肾，治疗健脾益气，养阴滋肾，扶正为主，兼以祛邪。提出以熄火、化瘀、补虚为分期论治大法，疾病发生和发展过程中，“火热”是主要病理因素，故熄火乃治疗首务；瘀血贯穿于疾病始终，是影响紫癜性肾炎发生及发展重要病理因素，故刘师强调临证有明显瘀血之证，定用活血化瘀通络法，然无明显瘀血征象，活血化瘀法亦不可偏废；“邪气所凑，其气必虚”，本病多先天禀赋不足，复感外邪，正胜则邪退，病情向愈，正虚则邪进，病情加重或反复发作，补虚有助于驱邪外出，有助于机体恢复，刘师主张补气为先并重视补益脾肾。
