小针刀联合TDP神灯治疗神经根型颈椎病随机平行对照研究

目的]观察小针刀联合TDP神灯治疗神经根型颈椎病疗效。方法]使用随机平行对照方法，将82例住院患者按随机数字表方法随机分为两组，对照组38例①电针（取穴：凤池、颈夹脊、大椎、肩井、阿是穴），取俯卧位，穴位皮肤常规消毒，30号1～2寸毫针对穴位依次进针，得气后接电针仪，以适宜为度，留针30min。1次／d，疗程间休息2d，②TDP神灯照射，30min／次，1次／d。治疗组44例小针刀，患者正坐低头趴于桌面或俯卧治疗床，头部尽量下垂，医生位于身后，右手拇食中指捏住刀柄，小针刀与颈部皮肤平面成90。垂直迅速刺入皮下，使刀口线和大血管、神经即肌肉纤维走向平行，针刀柄加压，使神经血管被分离在刀刃两侧。进针后颈椎痛点处找酸胀感，酸胀感处行平行于棘突纵向切开3～5刀，椎板和横突上横排切开3～5刀，黏连组织切开时术者手下有针刀落空感，术毕快速出针按压-3min防止出血。小针刀治疗处3d后给予TDP神灯照射治疗，疗法同对照组。连续治疗2周为1疗程。观测临床症状、不良反应。治疗1疗程，判定疗效。结果]治疗组痊愈30例，显效8例，有效4例，无效2例，总有效率95．50％。对照组痊愈20例，显效5例，有效6例，无效7例，总有效率81．60％。治疗组疗效优于对照垂且（P〈0．05）。结论]小针刀联合TDP神灯治疗神经根型颈椎病，疗效满意，值得推广。
