瘅痛酊与辣椒碱软膏分别联合西药治疗糖尿病周围神经病变随机平行对照研究

摘    要:[目的]观察瘅痛酊与辣椒碱软膏分别联合西药治疗糖尿病周围神经病变疗效。[方法]使用随机平行对照方法,将120例住院患者按随机数字表法分为三组。常规组(C组)40例控制饮食、适当运动、糖尿病教育、心理调节及口服降血糖药物或者皮下注射胰岛素使空腹血糖控制于〈8.0mmol/L,餐后血糖控制于8~10mmol/L。0.4g.kT.塞通+0.9%生理盐水100mL,1次/d,静滴。lmL甲钴胺+30mL的0.9%生理盐水,1次/d,静注。瘅痛酊组(A组)40例瘅痛酊,外擦患处(剂量视病变部位及范围而定),2次/d。西药治疗同常规组。辣椒碱组(B组)40例辣椒碱软膏外擦患处(剂量视病变部位及范围而定),2次/d。西药治疗同常规组。治疗20d为1疗程。观测临床症状、TCSS总积分、神经传导速度、不良反应。连续治疗1疗程(20d),判定疗效。[结果]瘅痛酊组基本缓解5例,显效12例,有效19例,无效4例,总有效率90.00%。辣椒碱组基本缓解3例,显效9例,有效23例,无效5例,总有效率87.50%。常规组基本缓解2例,显效4例,有效26例,无效8例,总有效率80.00%。临床疗效A组改善优于B、c组(P〈0.05)。神经传导速度三组均有改善(P〈0.05),A组改善优于B、c组(P〈0.05)。TCSS总积分方差分析三组均有改善(P〈0.05),A组改善优于B、c组(P〈0.05)。[结论]瘅痛酊与辣椒碱软膏分别联合西药治疗糖尿病周围神经病变效果显著,值得推广。

Abstract:[ Objective ] To observe the despair an illusion of pain tincture combined blood flux, a cobalt amine treating diabetic peripheral neuropathy effect. [ Methods ] Using random parallel control method, 120 cases of hospitalized patients into two groups according to random number table method. Routine group ( group C ) 40 cases control diet, proper exercise, diabetes education and psychological adjustment and oral fall blood sugar drugs or subcutaneous injections of insulin to fasting blood glucose control in 〈 8.0 tendency for L, postprandial blood glucose control in 8 -10 tendeney/L. 0.4 g blood plug+0.9% of the 100 ml saline, 1/d, static drops. 1 ml a cobalt amine+30 ml of 0.9% saline,1/d, the static note. Despair an illusion of pain tincture group ( group A ) despair an illusion of 40 patients pain tincture, outside the lint brush ( dose depends on the position and size of lesions ), 2 times/d. With conventional western medicine treatment group. Capsaicin group ( group B ) and 40 cases of eapsaicin ointment outside the lint brush ( dose depends on the position and size of lesions ), 2 times/ d. With conventional western medicine treatment group. Continuous treatment of 20 d 1 course of treatment. Observation of clinical symptoms, total integral TCSS, nerve conduction velocity, adverse reactions. Continuous treatment 1 treatment course ( 20d ), to determine efficacy. [ Results ] despair an illusion of basic relieve pain tincture group 5 cases, 12 cases were markedly effective, effective 19 cases, 4 cases were ineffective, the total effective rate was 90.00%. Capsaicin group basic ease 3 cases, 9 cases were markedly effective, 23 cases effectively, invalid in 5 cases, total effective rate was 87.50%. Conventional group basic ease in 2 cases, 4 cases had marked effect, effectively and 26 cases, 8 no effect, the total effective rate was 80.00%. Clinical curative effect of improved better than that of group B and C group A ( P〈0.05 ) . Nerve conduction velocity three groups were improved ( P〈0.05 ), group A is better than that of B and C group ( P〈0.05 ) . TCSS total score variance analysis of the three groups were improved ( P〈0.05 ), improve group A is better than that of B and C group ( P 〈0.05 ) . Despair an illusion of pain tincture. [ Conclusion ] Joint blood flux, a cobalt amine treating diabetic peripheral neuropathy effect is remarkable, is worth promoting.
