健脾止泻汤联合隔药灸治疗溃疡性结肠炎随机平行对照研究

[目的]观察健脾止泻汤联合隔药灸治疗溃疡性结肠炎疗效。[方法]使用随机平行对照方法，将74例住院患者按随机数字表分为两组。对照组34例柳氮磺胺吡啶片，1g／次，4次／d。治疗组40例①健脾止泻汤（党参15g，黄芪20g，柴胡、白芍、白术、柴胡各12g，枳实6g，香附、防风各10g，茯苓、山药各15g，蒲公英25g，薏苡仁18g，甘草10g），1剂／d，水煎300mL，早晚口服。②隔药灸（丹参，肉桂，附子，红花，木香，黄连）。选穴：主穴为天枢双、气海、中脘。辨证配穴：湿热蕴结加大肠俞双、水分；肝郁加太冲双；脾虚加脾俞双；肾阳虚加肾俞双、关元。施灸方法：药饼放于所选穴位，艾炷置于药饼上点燃，20min／穴。连续治疗3个月为1疗程。观测临床症状、免疫指标、不良反应。治疗1疗程，判定疗效。[结果始疗组痊愈5例，显效24例，有效6例，无效5例，总有效率87．50％。对照组痊愈2例，显效8例，有效12例，无效12例，总有效率67．70％。治疗组疗效优于对照组（P〈0．05）。免疫指标C3、LCT两组均有改善（P〈0．05，P〈0．01），治疗组改善优于对照组（P〈0．05）；IgG两组间无明显差异（P〉0．05）。[结论]健脾止泻汤联合隔药灸治疗溃疡性结肠炎，疗效满意，无副作用，值得推广。

The moxibustion （Salvia miltiorrhiza, cinnamon, aconite root, safflower, radix, Coptis ehinensis ） . Aeupoint selection： the main point of Tianshu Zhongwan, Qihai, double. Syndrome with acupuncture point： Damp heat accumulation with E. Yu Shuang, moisture ; liver depression and spleen deficiency and spleen Taiehong double; Yu Shuang; kidney yang deficiency and Shenshu double, Guan yuan. Method： medicinal cake moxibustion on the acupoints selected, ignite moxa cone is arranged in the drug on the cake, 20min/point. 3 months of continuous treatment of 1 courses. Observation of clinical symptoms, immune index, adverse reaction. 1 courses of treatment, the curative effect. [ Results ] Treatment group healed in 5 eases, 24 eases were markedly effective, 6 eases effective, invalid in 5 eases, total effective rate was 87.50%. The control group recovered in 2 cases, 8 eases were markedly improved, effective 12 cases, 12 cases ineffective, the total effective rate was 67.70%. The treatment group curative effect is better than that of control group （ P〈 0.05 ） . The immune indexes of C3 and LCT in the two groups were improved （ P〈 0.05, P〈 0.01 ）, the treatment group improved better than that of control group （ P〈 0.05 ） ; IgG no obvious difference between the two groups （ P〉 0.05 ） . [ Conclusion ] Jianpi Zhixie decoction combined with moxibustion for treatment of ulcerative colitis, satisfied with the effect, no side effect, is worth promoting.
