人工髋关节置换术假体柄周围骨折的原因分析与防治对策

目的回顾性分析人工髋关节置换术假体柄周围骨折的主要原因,探讨论决定预防措施及治疗方法。方法对本院近年收治的人工髋关节置换假体柄周围骨折的6例病例进行回顾性分析, 结果术后随访8-32个月,骨折愈合,假体固定可靠。结论骨质疏松、股骨近端解剖结构异常、假体型号选择不当是造成人工髋关节置换时假体柄周围骨折的主要原因,Vancouver假体柄周围骨折分型标准及治疗方案是较好的指导原则,术中应常规植骨。

Causes analysis and treatment of periprosthetic fraeture of the femur in total hip replacement

Objective To analysis the main cause, precaution and treatment of periprosthetic femur fracture in THR retrospectively. Methods Retrospective study was made on 6 causes of patients who have periprosthetic femur fractures. Result Through eight to thirty-two months follow-up, all the fracture were united , and in-corperaticon occured between the strut bone and host bone, The prosthesis stem fixed well, Conclusion Osteoporosis , the anatomical abnormal of proximal femur and inappropriate choice of prosthesis type attempting to maximize the degree of interference fit of prosthesis , were the main cause of periprosthetic femoral fracture in THR, Classifcation criteria and corresponding managements of periprosthetic fracture of Vancouver are proved to be suitable guide lines, Bone graft should be use regulaly.
