社区肺结核患者服药依从性的影响因素及对策

目的了解社区肺结核患者服药依从性的影响因素,建立社区结核病服药点的可行性。方法2005年1-12月对广州市越秀、荔湾区肺结核治疗中服药依从性差患者43例进行家庭访视,统一采用自行设计的调查问卷调查分析社区肺结核患者服药依从性的影响因素。结果患者认为交通费用高、交通不方便、工作不方便是主要影响因素,分别为25例(58%)、33例(77%)、29例(67%);提出设立方便患者的服药点31例(72%),建议加大财政帮助,改进DOTS管理系统,希望能改进就诊时间均为28例(65%)。结论交通不便、交通费用高、工作不方便等原因降低患者的服药依从性。利用社区卫生服务优质、价廉、便捷的特点设立社区服药点,方便社区肺结核患者服药,提高患者服药依从性,最终达到提高治愈率的目的。

The Influential Factors and the Countermeasures of Medicine-taking Compliance among Pulmonary Tuberculosis Patients in Community

Objective To understand the influential factors of medicine-taking compliance among pulmonary tuberculosis patients in community and probe into the feasibility of establishing medicine-taking stations for pulmonary tuberculosis patients in communities.Methods A self-designed questionnaire was used to survey 43 patients by home visits to understand the factors influencing their medicine-taking compliance during their therapy of anti-tuberculosis during Jan.to Dec.,2005.Results The main influential factors were thought to be high cost in traffic,inconvenience in traffic and inconvenience at work,accounting for 58%(25 cases),77%(33 cases),67%(29 cases),respectively.31 of them agreed on the establishment of medicine-taking stations,taking up 72%,28 of them suggested to increase financial aids,28 of them hoped improve the DOTS managing system and the same number of them wished to change the time to see doctors,taking up 65%,respectively.Conclusion Such factors as inconvenient traffic,high cost at traffic and inconvenience at work lower the compliance of taking medicine.Establishment of medicine-taking stations in the community using the quality service,cheap and convenient service facilities to facilitate their medicine taking can raise their compliance,ultimately reaching the goal of curing the disease.
