热疗联合化疗治疗中晚期非小细胞肺癌的临床观察

目的：探讨热疗联合化疗对中晚期非小细胞肺癌患者的治疗效果、不良反应和对生活质量的影响。方法：65例中晚期非小细胞肺癌患者，随机分为两组。治疗组：采用长春瑞滨（盖诺）＋顺铂方案同步在每一周期的化疗期间联合5次局部区域热疗共2个疗程；对照组：采用长春瑞滨（盖诺）＋顺铂方案化疗，2个周期后评价疗效、毒性反应及生存质量改善情况。结果：治疗组33例治疗后完全缓解2例，部分缓解19例，有效率63．6％；对照组32例治疗完后完全缓解2例，部分缓解10例，有效率37．5％。差异有显著性（P〈0．05）。生活质量改善率：治疗组66．7％，对照组37．7％，差异有显著性（P〈0．05）。毒性反应方面，两组最常见的血液学毒性为中性粒细胞减少，发生率分别为60％和68％无统计学差异（P〉0．05）。恶心、呕吐的发生率两组分别为51％和46％，无统计学差异（P〉0．05）。结论：热疗联合化疗治疗中晚期非小细胞肺癌有较好近期疗效，并能改善生活质量，而不增加毒性反应，值得临床进一步研究。

Clinical observation of thermotherapy in combination with chemotherapy in non-small cell lung cancer

Objective:To observe the efficacy,adverse effect and life quality in patients with advanced non small-cell lung cancer(NSCLC)treated with thermotherapy combined chemotherapy.Methods:Sixty five patients with advanced non small-cell lung cancer(NSCLC) randomized into two groups.The treatment group(33 patients)was received thermotherapy in conbination with chemotherapy.The control group(32 patients) underwent thermotherapy only.The therapeutic effect,adverse effect and life quality were evaluated after 2 cycles of therapy.Results:Two of 33 patients in intervention group achieved complete remission,19 patients achieved partial remission,63.6%(21/33) patients in intervention group response to thermotherapy combined chemotherapy,while 2 of 32 patients in control group achieved complete remission,10 patients achieved partial remission,37.5%(12/32) patients in control group response to thermotherapy,effective power was significant higher in combination group when compared to control group(P<0.05).The quality of life in treatment group was significantly better than that of control group(22/33)66.7% vs(12/32)37.7%,P<0.05.There was no significant difference in the incidence of complications between two groups,which mainly included neutrocytopenia(60% vs 68%,P>0.05),nausea and vomiting(51% vs 46%,P>0.05).Conclusion:Thermotherapy in combination with chemotherapy are effective in the treatment of NSCLC.It can improve the quality of the patients' life and the side effects can be tolerated.
