人工关节置换与内固定治疗老年骨质疏松性不稳定型股骨粗隆间骨折的效果比较

目的探讨人工关节置换与内固定治疗老年骨质疏松性不稳定型股骨粗隆间骨折的临床效果。方法选择本院2010年1月~2013年1月收治的老年骨质疏松性不稳定型股骨粗隆间骨折患者41例为研究对象,按照治疗方式不同分为两组,对照组给予内固定治疗,观察组则给予人工关节置换治疗,观察两组临床治疗效果。结果除术中出血量外,观察组手术时间、下床活动时间及住院时间均优于对照组,差异有统计学意义（P〈0.05）;观察组治疗总有效率为86.36%,明显高于对照组的68.42%,差异有统计学意义（P〈0.05）;观察组并发症发生率为9.09%,明显低于对照组的36.84%,差异有统计学意义（P〈0.05）。结论人工关节置换治疗老年骨质疏松性不稳定型股骨粗隆间骨折的临床效果显著,能够有效帮助患者提高生活质量,值得临床推广。

Clinical efficacy of artificial joint replacement and internal fixation in the treatment of elderly osteoporosis patients with unstable femoral intertrochanteric fracture

Objective To investigate clinical efficacy of artificial joint replacement and internal fixation in the treatment of elderly osteoporosis patients with unstable femoral intertrochanteric fracture.Methods 41 cases of elderly osteoporosis patients with unstable femoral intertrochanteric fracture admitted to our hospital from January 2010 to January 2013 were selected as research objects,and divided into two groups according to different methods,the control group was given internal fixation and observation group was given artificial joint replacement,the efficacy of two groups were compared.Results In addition to intraoperative blood loss,the operation time,ambulation time and hospital stay of observation group were better than that of control group,the difference was significant（P0.05）;the total efficiency rate of observation group（86.36%）was higher than that of control group（68.42%）,the difference was significant（P0.05）;the complication rate of observation group（9.09%）was lower than that of control group（36.84%）,the difference was significant（P 0.05）.Conclusion Artificial joint replacement has significant effect on treating elderly osteoporosis patients with unstable femoral intertrochanteric fracture,can effectively help patients improve the quality of life,is worthy of clinical application.
