Turner氏综合征的临床与骨改变分析(附15例报告)   总被引:2,自引:0,他引:2  
目的通过对15例有详细的临床及影像资料的Turner氏综合征分析,探讨先天性卵巢发育不良综合征(Turner)的临床与骨改变特点。方法收集15例Turner氏综合征临床及骨骼X线平片,分析其表现。结果染色体核型分3型,第1型45,X,8例;第2型46,XX,5例;第3型46,XX/45,X,2例。X线表现有:骨质稀疏14例,掌骨征阳性8例,指骨优势9例,肘外翻10例,脊柱骨化不良5例,足趾趾节短9例,颅底凹陷11例。结论Turner氏综合征患者最终身高明显低于正常人群,智力低下,性发育不全。X线表现典型,骨改变对临床的诊断具有主要参考价值。  相似文献

胫骨高位截骨手术并发症23例次分析   总被引:2,自引:1,他引:1  
[目的] 探讨胫骨高位截骨手术并发症的发生情况,并提出预防和治疗措施。[方法]2000年1月~2004年10月采用胫骨高位截骨术治疗膝骨关节炎合并内翻畸形患者126人,21人发生手术并发症,男4例,女17例;年龄48~64岁,平均61岁。术前拍摄站立膝关节正位X线片,测量股骨一胫骨角,计算截骨角度,采用外侧闭合胫骨高位截骨术矫正膝内翻畸形。[结果] 术后随访6~12个月,平均7.5个月。共21名患者发生各类并发症23例次.发生率为16.7%。其中发生胫骨骨折4例,腓总神经麻痹3例,出现深静脉血栓形成5例,膝内翻复发病例6例,内固定失败4例(其中2例合并膝内翻复发),感染1例。[结论] 降低胫骨高位截骨手术并发症需要术者熟悉局部解剖和精确的术前设计,提高手术技巧及完善的围手术期护理。  相似文献

High tibial osteotomy (HTO) has traditionally been used to treat varus gonarthrosis in younger, active patients. Varus malalignment increases the risk of progression of medial compartment osteoarthritis and an HTO can be performed to realign the mechanical axis of the lower limb towards the lateral compartment, thereby decreasing contact pressures in the medial compartment. Anterior cruciate ligament (ACL) insufficiency may lead to post-traumatic arthritis due to altered joint loading and associated injuries to the menisci and articular cartilage. Understanding the importance of posterior tibial slope and its role in sagittal knee stability has led to the development of biplane osteotomies designed to flatten the posterior tibial slope in the ACL deficient knee. Altering the alignment in both the sagittal and coronal planes helps improve stability as well as alter the load in the medial compartment. Detailed history, physical exam and radiographic analysis guide treatment decisions in this high demand patient population. Lateral closing wedge (LCW) and medial opening wedge (MOW) HTOs have been performed and their potential advantages and disadvantages have been well described. Given the triangular shape of the proximal tibia, it is imperative that the surgeon pay close attention to the geometry of the osteotomy “gap” when performing MOW HTO to avoid inadvertently increasing the posterior tibial slope. Simultaneous ACL reconstruction may require technique modifications depending on the type of HTO and ACL graft chosen. With appropriate patient selection and good surgical technique, it is reasonable to expect patients to return to activities of daily living and recreational sports without debilitating pain or instability.  相似文献

This cross-sectional study investigated (i) the association of varus thrust during gait with the presence of patellofemoral osteoarthritis (PFOA) in patients with medial knee osteoarthritis (OA) and (ii) patellar alignment in the knees with varus thrust. Participants from orthopedic clinics (n = 171; mean age, 73.4 years; 71.9% female) diagnosed with radiographic medial knee OA (Kellgren/Lawrence [K/L] grade ≥1) were included in this study, and underwent gait observation for varus thrust assessment using 2D video analysis. A radiographic skyline view was used to assess the presence of medial PFOA using the grading system from the Osteoarthritis Research Society International Atlas. The tibiofemoral joint K/L grade, patellar alignment (i.e., lateral shift and tilting angle), and knee pain intensity were also evaluated as covariates. Thirty-two (18.7%) of 171 patients exhibited varus thrust and they presented significantly higher knee pain (46.0 ± 3.04 mm vs. 32.4 ± 2.73 mm; P = 0.024), a lower patellar tilting angle (P = 0.024), and a higher prevalence of PFOA compared with those without varus thrust. A logistic regression analysis with adjustment of covariates showed that varus thrust was significantly associated with higher odds of the presence of mixed and medial PFOA, and trended to significantly associate with any PFOA, including lateral PFOA. This indicates that varus thrust was associated with PFOA in a compartment-nonspecific manner in patients with medial knee OA. Varus thrust may represent a clinical disease feature of more advanced and multicompartmental disease.  相似文献

Ankle sprains are one of the most common soft tissue injuries accounting for nearly 40% of sports injuries.There are large number of procedures for its treatment reported in the literature with largely good results.The ankle forms a functional unit with the subtalar joint.We present a new classification for peritalar lateral instability.There are two intents of this classification. Firstly, the classification demonstrates an assessment and treatment guideline for the many causes of peritalar lateral instability. The second use of the classification is for research purposes so that cohorts of patients can be accurately described and the efficacy of different operations in different groups can be properly assessed.  相似文献

BackgroundGeneral ankle-foot orthosis (AFO) cannot be flexibly adjusted to volumetric change in the lower leg because the molding is custom-fit. Thus, we developed a flexible assistive device called elastic neutral AFO (EN-AFO) to help stroke patients hold a neutral ankle position. The purpose of this study was to investigate the effects of EN-AFO and improve gait patterns in stroke patients with rearfoot varus deformity.MethodsFifteen stroke patients with a varus deformity of the foot performed a walking test with and without the use of EN-AFO. Kinematic data were measured with a 3D motion analysis system with inertial measurement unit (IMU) sensors.ResultsIn the stance phase, maximal pelvic tilt and maximal ankle dorsiflexion in the affected side changed, and maximal and minimal pelvic tilts and maximal hip abduction in the less-affected limb effectively changed, as well. During the swing phase, minimal pelvic tilt and minimal ankle abduction in the affective limb greatly changed; particularly, the reduction of maximal ankle inversion was significantly cleared.ConclusionsThe EN-AFO was effective in controlling the tendency of foot inversion in patients with varus deformities. This is suitable for gait training, as it can adjust the orthosis stiffness according to the foot condition.  相似文献

背景:交叉克氏针内固定是目前治疗儿童GartlandⅢ型肱骨髁上骨折的首选方法,但仍未从根本上解决并发症发生问题。目的:评价尺神经沟置针术式治疗儿童GartlandⅢ型肱骨髁上骨折的可行性及疗效。 方法:1998年1月到2010年10月采用尺神经沟置针术式治疗儿童GartlandⅢ型肱骨髁上骨折患儿144例,随访时检查环小指运动及感觉情况,测量双上肢长度、肘关节活动度,摄双肘正位片测量Baumann角或提携角。采用HSS肘关节功能评价法评分标准进行肘关节功能评定。 结果:144例患儿,随访14~85个月,平均60个月,根据HSS肘关节功能评定法行功能评价:优140例,良4例,无肘内翻和肘外翻畸形发生,无继发尺神经损伤,肢体无短缩。 结论:尺神经沟置针术式治疗儿童GartlandⅢ型肱骨髁上骨折可获得良好的肘关节功能及外观,预防了肘内翻畸形及继发尺神经损伤,且对骨骺生长发育无明显影响。  相似文献

Background

During ligament balancing for severe medial contracture in varus knee total knee arthroplasty (TKA), complete distal release of the medial collateral ligament (MCL) or a medial epicondylar osteotomy can be necessary if a large amount of correction is needed.

Methods

This study retrospectively reviewed 9 cases of complete distal release of the MCL and 11 cases of medial epicondylar osteotomy which were used to correct severe medial contracture. The mean follow-up periods were 46.5 months (range, 36 to 78 months) and 39.8 months (range, 32 to 65 months), respectively.

Results

There were no significant differences in the clinical results between the two groups. However, the valgus stress radiograph revealed significant differences in medial instability. In complete distal release of the MCL, some stability was obtained by repair and bracing but the medial instability could not be removed completely.

Conclusions

Medial epicondylar osteotomy for a varus deformity in TKA could provide constant medial stability and be a useful ligament balancing technique.  相似文献
