Circulating endothelial microparticles: a new marker of vascular injury

Cell activation or apoptosis leads to plasma membrane blebbing and microparticles (MPs) release. MPs are submicron membrane vesicles expressing a panel of oxidized phospholipids and proteins specific of the cells they originate from. Exposure of negatively charged phospholipids and tissue factor confers a procoagulant potential to MPs. Increases in plasma MPs levels, particularly those of endothelial origin, reflects cellular injury and appears now as a surrogate marker of vascular dysfunction. MPs are also biologically active and stimulate pro-inflammatory responses in target cells. Thus, MPs can promote a prothrombogenic and pro-inflammatory vicious circle leading to vascular dysfunction. A better understanding of MPs composition, as well as their effects and the mechanisms leading to their clearance will likely open new therapeutic approaches in the treatment and the prognosis of cardiovascular diseases.     

Tumeurs épithéliales thymiques : actualités dans la prise en charge en France

Thymic epithelial neoplasms are rare malignancies with about 250 new incident cases in France every year. The WHO histologic classification distinguishes thymoma and thymic carcinoma which are tumors with different biological and clinical behaviors and outcomes. The Masaoka-Koga staging system is considered as a reference and is also of prognosis value. Diagnosis, multimodal treatment and follow-up of thymic epithelial neoplasms require a multidisciplinary approach where surgery is the cornerstone treatment. A national expert center coordinates thymic epithelial neoplasms management with 12 other regional expert centers through the French organization named RYTHMIC (www.rythmic.org). Patient's files have to be discussed at regional or national multidisciplinary staff. A group of expert pathologists will centrally review tumors when the diagnosis or classification is a matter of controversy. Among its objectives, RYHTMIC has to promote medical education, patient's information and research. This review focuses on RYTHMIC guidelines and data regarding multimodal management and targeted therapies in epithelial thymic neoplasms.     

Lymph node supraclavicular metastasis of thymic carcinoma: an inaugural and unusual feature of a rare disease

Among the epithelial tumours of the thymus, thymic carcinomas are rare, involving more aggressive locoregional behaviour and a poor prognosis. More than one-third of cases present lymph node metastasis during an evaluation of the extension of a primitive mediastinal tumour of thymic origin. The authors report an unusual case in a 46-year-old woman presenting a metastatic form. The diagnosis of epidermoid thymic carcinoma was based on the histopathological examination of a right supraclavicular lymph node, associated with an anterior radiological mediastinal tumoral syndrome, raising the delicate problem of its diagnosis and treatment.
