Summary
Early recognition of recurrence and work-up of clinically indeterminate lesions may be impaired after reconstruction with
silicone implants due to superimposition of the implant or to scarring. This study was undertaken to evaluate the use of contrast-enhanced
MRI in patients with silicone implant after breast cancer. Contrast-enhanded MRI was offered to 169 patients. Comparative
two- to three-view mammography was also performed in 169 patients, as well as comparative sonography in 144 patients. Conventional
imaging and clinical examination detected only 8/13 recurrences, whereas 12/13 were detected by MRI. One recurrence had been
visible as a strongly enhancing 2-mm dot in a previous examination (2 years before), but was not called. It was therefore
counted as false negative. In addition, multicentricity was detected by MRI alone in two of three cases. MRI correctly diagnosed
scar tissue in all cases with indeterminate findings. However, due to false-positive calls caused by enhancing granulomas
specificity could not be improved. Contrast-enhanded MRI allowed decisive additional information in our study group and improved
the sensitivity significantly (concerning all diagnoses). Contrast-enhanded MRI is recommended in patients with diagnostic
problems or high risk of recurrence after silicone implants.

方法:12只新西兰白兔的24只眼,随机分为3组,行气体压迫玻璃体手术3天后,分别向玻璃体腔内注入硅油(4只眼)、5μg/ml维甲酸硅油(10只眼)、10μg/ml维甲酸硅油(10只眼)各
0.5m1,用检眼镜、光漳和电镜检查来观察视网膜变化情况。
结果:注入硅油28天后,未发现各浓度的维甲酸硅油对视网膜产生毒性作用。结论:浓度为5、10μg/ml的维甲酸硅油注入玻璃体腔4周,对视网膜不产生毒性作用。

Breast augmentation is one of the most frequently performed cosmetic surgery procedures worldwide. We report on a series of 502 consecutive cosmetic breast augmentations by a single surgeon, using both the submuscular and submammary approaches. A vacuum drain was placed in the surgical pocket at the end of the procedure. The drain fluid collection was recorded immediately after the surgery and the following day before removal. A tenfold increase in the amount of fluid collection was noted in the drain’s reservoir. Thirteen patients presented with minor wound healing problems at the second postoperative follow-up, which were treated conservatively; two of them had the implant removed and replaced after 2 months. There were only two capsular contractions at 1-year follow-up. We focus our discussion on the drainage in the early postoperative period and conclude that the routine use of a surgical drainage reduces the risk for complications. Based on our findings, we advocate that short-term routine use of a surgical drain is advisable. 相似文献

Solid customized and prefabricated silicone implants have been used by the author for 15 years in a wide range of chest wall
deformities. Chest wall implants are often used in males seeking to augment a muscularly deficient or underdeveloped chest;
however, their greatest use has come in a variety of deformities both congenital and acquired, such as pectus excavatum, Poland's
Syndrome, and pectoralis muscle tears. The implants can be either customized using a moulage technique or are prefabricated,
manufactured implants which can be modified on the operating table to repair the contour deformity. The immediate postoperative
problem of seroma and subcutaneous implant ``show' has been minimized by careful planning, gentle technique, deep insertion,
improved patient positioning on the operating room table, and the use of oral anti-inflammatory medications. The long-term
results of these implants seem very satisfactory. The patients are usualy physically active, and the implants show no long-term

There is a growing need for research into new skin- and environment-friendly surfactants. This paper focuses on a natural surfactant of an alkylpolyglucoside type, which can form both thermotropic and lyotropic liquid-crystalline phases. The aim of this study was to relate some physicochemical properties (characterised by polarisation and transmission electron microscopy, thermal analysis and rheology) of the three formulations based on cetearyl glucoside and cetearyl alcohol, to the results of in vitro and in vivo bioavailability of hydrocortisone (HC). The three formulations contained oils of different polarity (medium chain triglycerides: MG, isopropyl myristate: IPM and light liquid paraffin: LP), respectively. In vitro permeation was followed through the artificial skin constructs (ASC), while the parameters measured in vivo were erythema index: EI (using instrumental human skin blanching assay), transepidermal water loss (TEWL) and stratum corneum hydration (SCH). The vehicles based on cetearyl glucoside and cetearyl alcohol showed a complex colloidal structure of lamellar liquid-crystalline and lamellar gel-crystalline type, depending on oil polarity. Rheological profile of the vehicle was directly related to the in vitro profile of the HC permeation. In vivo results suggested that the vehicle with MG retarded the HC permeation, whereas less polar IPM and non-polar LP enhanced it. It is suggested that the enhancement is achieved either by a direct interaction with lipid lamellae of the SC or indirectly by improving skin hydration.
There were no adverse effects during in vivo study, which indicates a good safety profile of this alkylpolyglucoside surfactant. 相似文献

The pectus excavatum deformity is characterised by a deep depression usually involving the lower one-half to two-thirds of the sternum. The indications for surgery are often aesthetic. Extensive procedures, requiring fracturing and remodelling of the chest wall skeleton are associated with high morbidity and high rate of complications. In this article we describe our renewed experience with reconstruction of mild and moderate pectus excavatum deformities with custom made prefabricated silicone implants. The fabrication of the implant and the surgical technique are described in detail. An excellent aesthetic correction of the deformity was achieved in all of the patients in our series, with high patient satisfaction rate. We conclude that with careful patient selection, artistic implant fabrication and meticulous surgical technique, this approach achieves excellent aesthetic correction with minimal morbidity and a low complication rate and therefore should maintain its place in the armamentarium of surgical techniques for reconstruction of pectus deformities. 相似文献

The effects of linoleic acid, linoleic acid anilide, and arachidonic acid on the expression of CD11b/CD18, CD11c/CD18 integrins
and l-selectin on human neutrophils were studied by flow cytometry in a whole blood assay. None of these compounds had any effect
on the basal expression of CD11b, CD11c, or l-selectin in the concentration range of 20–100 μM. However, linoleic acid at a concentration of 1000 μM slightly up-regulated
CD11b and CD11c by a factor of 2.1 and 1.7, respectively. Linoleic acid, linoleic acid anilide, and arachidonic acid did not
affect the formyl-methionyl-leucyl-phenylalanine induced up-regulation of CD11b or CD11c. However, linoleic acid and linoleic
acid anilide slightly inhibited the phorbol myristate acetate (PMA)-induced expression of CD11b, which was decreased by 27
and 21% at concentrations of 100 and 1000 μM, respectively. Likewise, arachidonic acid at 40 μM inhibited the PMA-induced
expression of CD11b by 19%. Our results suggest that linoleic acid, linoleic acid anilide, and arachidonic acid do not dramatically
affect the expression of leukocyte adhesion molecules in a whole blood assay.
