目的:探讨慢性阻塞性肺疾病(COPD)呼吸机依赖患者膈肌起搏器的应用及疗效。方法:把2018年6月~2019年6月本院呼吸内科收治的COPD使用呼吸机并发生依赖的清醒患者60例作为研究对象,男27例,女33例;患者年龄60~75岁,平均(66.3±1.2)岁,随机分为观察组(运用常规治疗及护理并增加使用膈肌起搏器)和对照组(运用常规治疗及护理),每组30例,对两组脱机成功率和脱机时间进行比较。结果:观察组患者脱机情况明显优于对照组,观察组与对照组2周脱机成功率分别为83.3%和46.7%,脱机时间分别为(7.56±1.45),(9.42±1.28)d,差异均有统计学意义(P<0.05)。结论:COPD呼吸机依赖患者准备脱机前,给予膈肌起搏器治疗,可明显提高脱机成功率及缩短呼吸机使用时间。  相似文献

目的:探讨分析键对键与面对面联动在改善心外科患者不良情绪和睡眠质量中的应用效果.方法:选取2018年5月至2019年5 月湖南师范大学附属第一医院收治的心脏外科手术患者100例作为研究对象,按照患者病房号分为对照组和观察组,每组50例.对照组采用常规护理,观察组在此基础上实施键对键与面对面联动的护理模式.结果:出院3个月后,与对照组患者比较,观察组患者情绪、机体不适感、睡眠质量显著改善,差异均有统计学意义(P<0.05).结论:键对键与面对面联动的护理模式可显著改善心外科患者的不良情绪,提高睡眠质量,值得临床推广.  相似文献

PurposeThe introduction of a daily image-guided radiation therapy (IGRT) program is an important step. It has implications for the radiation therapy team in terms of accuracy, workflow, and decision making. This study assesses how successful a radiation therapy department has been in using this technology and the accuracy of individual decision making when comparing online and offline image match data.MethodsTwenty intensity-modulated radiation therapy head and neck patients had their IGRT data assessed retrospectively. Online analysis was completed based on a 0-mm action threshold. Offline analysis was then conducted on the same data. Any discrepancies in decision making were then assessed.ResultsResults indicated that the treating radiation therapy team was able to image match consistently when benchmarked against their colleagues in the offline environment. Analysis of online versus offline corrections in each of the three orthogonal directions showed strong agreement. Further analysis revealed no statistically significant differences for systematic errors, whereas a statistically significant but small difference was present for random error.ConclusionIn this age of sophisticated equipment, daily IGRT is a valuable modality. However, the introduction of daily online IGRT inclusive of a 0-mm action threshold for head and neck IGRT requires careful consideration and evidence that such accuracy can be achieved. Ultimately, it is still the radiation therapist who must make the decision, which places great importance on the competency of the treating radiation therapy team.  相似文献

Background: Despite the increasing popularity of video game playing, little is known about the similarities and differences between online and offline video game players. Objectives: The aims of this study were (i) to test the applicability and the measurement invariance of the previously developed Problematic Online Gaming Questionnaire (POGQ) in both online and offline gamers and to (ii) examine the differences in these groups. Methods: Video game use habits and POGQ were assessed in a sample of 1,964 (71% male) adolescent videogame players. Those gamers who played at least sometimes in an online context were considered “online gamers,” while those who played videogames exclusively offline were considered “offline gamers.” Results: Confirmatory factor analysis supported the measurement invariance across online and offline videogame players. According to the multiple indicators multiple causes (MIMIC) model, online gamers were more likely to score higher on overuse, interpersonal conflict, and social isolation subscales of the POGQ. Conclusion: The results of the present study suggest that online and offline gaming can be assessed using the same psychometric instrument. These findings open the possibility for future research studies concerning problematic video gaming to include participants who exclusively play online or offline games, or both. However, the study also identified important structural features about how online and offline gaming might contribute differently to problematic use. These results provide important information that could be utilized in parental education and the prevention program about the possible detrimental consequences of online vs. offline video gaming.  相似文献

目的:结合临床应用需求,研制一种新式混合型生物人工肝治疗应用系统。方法:以2个可编程控制器(PLC)作为控制系统核心,辅以多种安全监测报警装置,具有密闭、洁净和恒温的生物反应治疗环境,为离线混合型生物人工肝治疗提供一体化集成解决方案。结果:该设备能实现对血液流量、流速、质量等参数的精确控制,并提供温度、气泡、漏血等参数的报警监测功能。控制系统操作简便,人机界面友好,并为封闭式生物反应离线治疗过程提供实时影像监控。结论:新型治疗设备实现了混合式生物反应人工肝的离线治疗,有效缩短了生物人工肝治疗时间,同时改善了生物治疗环境。  相似文献

When people are confronted with feedback that counters their prior beliefs, they preferentially rely on desirable rather than undesirable feedback in belief updating, i.e. an optimism bias. In two pre-registered EEG studies employing an adverse life event probability estimation task, we investigated the neurocognitive processes that support the formation and the change of optimism biases in immediate and 24 h delayed tests. We found that optimistic belief updating biases not only emerged immediately but also became significantly larger after 24 h, suggesting an active role of valence-dependent offline consolidation processes in the change of optimism biases. Participants also showed optimistic memory biases: they were less accurate in remembering undesirable than desirable feedback probabilities, with inferior memories of undesirable feedback associated with lower belief updating in the delayed test. Examining event-related brain potentials (ERPs) revealed that desirability of feedback biased initial encoding: desirable feedback elicited larger P300s than undesirable feedback, with larger P300 amplitudes predicting both higher belief updating and memory accuracies. These results suggest that desirability of feedback could bias both online and offline memory-related processes such as encoding and consolidation, with both processes contributing to the formation and change of optimism biases.  相似文献

目的 观察中西医结合对重症肺炎机械通气患者脱机的影响.方法 对90例重症肺炎合并呼吸衰竭进行机械通气治疗的患者进行随机分组,每组45例,治疗组给予中西医联合治疗,对照组给予常规西医治疗,比较两组患者脱机效果.结果 治疗前两组患者VT、f、HR及PaCO2水平比较无明显差异(P〉0.05),治疗21天后各项指标均较治疗前有改善(P〈0.05),但同期比较治疗组改善更明显(P〈0.05);治疗组VAP发生率、机械通气及ICU住院时间均显著短于对照组(P〈0.01),具有统计学意义.结论 中西医结合法对重症肺炎机械通气患者脱机有积极影响.  相似文献

Aims: The aim of the current study was to investigate agrammatic IWAs' online and offline sentence comprehension simultaneously in order to reveal what online sentence-processing strategies they rely on and how these differ from controls' processing routines. We further asked whether IWAs' offline chance performance for non-canonical sentences does indeed result from guessing.

Methods & Procedures: We used the visual-world paradigm and measured eye movements (as an index of online sentence processing) of controls (N?=?8) and individuals with aphasia (N?=?7) during a sentence–picture matching task. Additional offline measures were accuracy and reaction times.

Outcomes & Results: While the offline accuracy results corresponded to the pattern predicted by the TDH, IWAs' eye movements revealed systematic differences depending on the response accuracy.

Conclusions: These findings constitute evidence against attributing IWAs' chance performance for non-canonical structures to mere guessing. Instead, our results support processing deficit explanations and characterise the agrammatic parser as deterministic and inefficient: it is slowed down, affected by intermittent deficiencies in performing syntactic operations, and fails to compute reanalysis even when one is detected.  相似文献

Consolidation of motor memories associated with skilled practice can occur both online, concurrent with practice, and offline, after practice has ended. The current study investigated the role of dorsal premotor cortex (PMd) in early offline motor memory consolidation of implicit sequence‐specific learning. Thirty‐three participants were assigned to one of three groups of repetitive transcranial magnetic stimulation (rTMS) over left PMd (5 Hz, 1 Hz or control) immediately following practice of a novel continuous tracking task. There was no additional practice following rTMS. This procedure was repeated for 4 days. The continuous tracking task contained a repeated sequence that could be learned implicitly and random sequences that could not. On a separate fifth day, a retention test was performed to assess implicit sequence‐specific motor learning of the task. Tracking error was decreased for the group who received 1 Hz rTMS over the PMd during the early consolidation period immediately following practice compared with control or 5 Hz rTMS. Enhanced sequence‐specific learning with 1 Hz rTMS following practice was due to greater offline consolidation, not differences in online learning between the groups within practice days. A follow‐up experiment revealed that stimulation of PMd following practice did not differentially change motor cortical excitability, suggesting that changes in offline consolidation can be largely attributed to stimulation‐induced changes in PMd. These findings support a differential role for the PMd in support of online and offline sequence‐specific learning of a visuomotor task and offer converging evidence for competing memory systems.  相似文献
