目的 探讨一次性根管与常规根管治疗后冠修复对隐裂性牙髓炎患者的治疗效果。方法 选取2014年5月至2017年5月诊治的187例隐裂性牙髓炎患者随机分为观察组和对照组,观察组给予一次性根管治疗后冠修复,对照组给予常规根管治疗后冠修复,比较两组术前及术后1周Mohd Sulong疼痛分级、术后3个月临床疗效、术后6个月X线改变、术后2年治疗成功率。结果 术前两组Mohd Sulong疼痛分级无统计学差异(P>0.05),冠修复术后1周两组疼痛分级无统计学差异(P>0.05),术后3个月观察组临床疗效总有效率为97.83%较对照组88.41%显著提高(P<0.05),术后6个月观察X线改变率为3.26%较对照组14.74%显著降低(P<0.05),术后2年观察组治疗成功率为94.57%较对照组82.11%显著提高(P<0.05)。结论 一次性根管治疗后冠修复能够缓解隐裂性牙髓炎疼痛,提高临床疗效,提高患者咀嚼功能和生活质量,值得在临床上推广。  相似文献

A forty year old female patient developed paraesthesia of the right side of her lower lip following the placement of an extensive pin-retained amalgam restoration in her lower right second premolar tooth. Radiographs indicated that the mental foramen was close to the apex of this tooth and it was assumed that postoperative pulpitis and periapical inflammation had caused the paraesthesia through the effects of pressure on the mental nerve. The paraesthesia resolved following endodontic treatment of the lower second premolar tooth and the patient has had no further signs or symtoms.  相似文献

目的:比较不同充填方法在乳牙中牙髓炎治疗效果.方法:序贯对100颗乳牙牙髓炎随机分为2组,分别用根管充填和干髓治疗两种方法.结果:氧化锌丁香油根管充填术治疗组成功率为92%,而干髓术治疗乳牙牙髓炎成功率仅为48%.结论:根管充填术较干髓术疗效更肯定,减少患儿群体复诊数和人均复诊次数,有利于临床推广.  相似文献

IntroductionThe current systematic review and meta-analysis aimed to evaluate the success rate of partial pulpotomy in treating permanent posterior teeth with carious vital pulp exposure. A secondary aim was to assess the prognostic factors using a meta-regression.MethodsAn electronic search was performed for studies from January 1950 to November 2018 in the following databases: PubMed, ScienceDirect, and Cochrane. All searches were performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Clinical studies evaluating the success rate of cariously exposed vital human permanent posterior teeth treated with a partial pulpotomy were selected. Only randomized clinical trials and prospective clinical studies were included for evaluation. The Newcastle-Ottawa Scale and the Cochrane Collaboration’s tool were used to evaluate risk assessment.ResultsFrom the 218 studies identified through the initial search, 11 studies qualified for the final analysis (5 randomized clinical trials and 6 prospective studies). The results of the meta-analysis indicate a success rate of 98% (confidence interval [CI]: 0.94–1), 96% (CI: 0.92–0.99), and 92% (CI: 0.83–0.97) after 6 months and 1 and 2 years of follow-up. Examining the probable prognostic factors using meta-regression analysis, only preoperative pulp status (P = .001) was identified as a significant factor, with studies including teeth with the presumptive diagnosis of irreversible pulpitis displaying significantly lower results. The final solution, pulp capping material, apex closure, and the age of the patient did not affect the treatment success rate (P > .05).ConclusionsThe available data suggest that a partial pulpotomy results in high success rates in treating cariously exposed permanent posterior teeth up to 2 years. Six months of monitoring can be considered an appropriate period when evaluating the success of a partial pulpotomy although more clinical and radiographic controls are essential to ensuring success.  相似文献

IntroductionPrevious studies have shown that in teeth presenting with symptoms of irreversible pulpitis (IP), bacteria and their by-products driving inflammation are confined mainly within the coronal pulpal tissue. The present study aimed to determine the presence and identity of bacteria within pulps presenting with clinical symptoms of IP using molecular methods.MethodsSamples were obtained from 30 adult patients presenting to the dental emergency department with signs and symptoms of IP. After meticulous surface decontamination, the pulp space was accessed, and clinical samples were collected from inflamed pulp tissue using sterile paper points. Genomic DNA was extracted from the clinical samples, and quantification of bacteria was performed using quantitative polymerase chain reaction targeting the conserved 16S ribosomal RNA (rRNA) gene. To characterize the microbial composition, the V3–V5 hypervariable regions of the 16S rRNA gene were amplified and subjected to next-generation sequencing on the MiSeq platform (Illumina, San Diego, CA).ResultsOf the 30 teeth that presented with IP, half of the intracanal samples had a substantial bacterial load (16S rRNA copies) within the IP vital pulp as determined by quantitative polymerase chain reaction. Next-generation sequencing microbial identification was successful in 7 intracanal samples and yielded 187 bacterial operational taxonomic units within the IP samples. The most abundant genera observed among the vital cases were Veillonella (16%), Streptococcus (13%), Corynebacterium (10%), Cutibacterium (9.3%), and Porphyromonas (5.7%).ConclusionsThe current study highlighted the evidence of vital teeth diagnosed as IP harboring considerable bacterial loads and composed of genera reflective of established endodontic pathology and thus may offer insights into the initial events preceding pulpal necrosis.  相似文献

Objective To evaluate the effectiveness and safety of a computer-controlled periodontal ligament (PDL) injection system to the local soft tissues as the primary technique in endodontic access to mandibular posterior teeth in patients with irreversible pulpitis. Methods A total of 162 Chinese patients who had been diagnosed with irreversible pulpitis in their mandibular posterior teeth without acute infection or inflammation in the periodontal tissues were enrolled in this clinical study. The patients were divided into 3 groups according to the position of the involved tooth: the premolar group (PM, z2=38), first molar group (FM, n=66), and second molar group (SM, n=58). All the patients received computer-controlled PDL injection with 4% articaine and 1 ' 100 000 epinephrine. Immediately after the injection, endodontic access was performed, and the degree of pain during the treatment was evaluated by the patients using Visual Analogue Scale for pain. The success rates were compared among the 3 groups. The responses of local soft tissues were evaluated 3-8 days and 3 weeks after the procedure. Results The overall success rate was 76.5%. There was a significant difference in success rates among the PM, FM, and SM groups (92.1%, 53.0%, 93.1%, respectively; 2 X =34.3, P〈0.01). Both the PM and SM groups showed higher success rates than that of the FM group (v=l, f=16.73, P〈0.01, v=l, Z = 4.5, X2 2 P〈0.01). No irreversible adverse effects on the periodontal soft tissues at the injection sites were observed in the follow-up visits in any of the groups. Conclusion The computer-controlled PDL injection system demonstrates both satisfactory anesthetic effects and safety in local soft tissues as primary anesthetic technique in endodontic access to the mandibular posterior teeth in patients with irreversible pulpitis.  相似文献

目的:系统评价一次性根管治疗对急性牙髓炎的近期和远期疗效。方法:计算机检索Cochrane图书馆数据库,Pub Med,中国生物医学文献数据库(CBMdisc),中国期刊全文数据库(CNKI)和维普期刊数据库(VIP)发表的关于一次性根管治疗和急性牙髓炎的文献,查找的时间从建库到2014年5月20日。按纳入和排除的标准来遴选文献,并根据Cochrane评价员手册5.0评估纳入研究的偏倚风险。采用Rev Man5.0软件对纳入文献进行统计分析。结果:共纳入13篇临床随机对照研究,Meta分析结果显示一次性根管治疗的近期总有效率优于多次性根管治疗[OR=2.91,95%CI(2.00,4.24)],同时,一次性根管治疗的远期总有效率也优于多次性根管治疗[OR=2.59,95%CI(1.65,4.06)]。结论:现有研究显示一次性根管治疗急性牙髓炎有临床疗效。  相似文献

IntroductionMolecular diagnosis may overcome the limitations of clinical and histologic diagnosis in pulpitis, thereby benefiting many treatment techniques, such as vital pulp therapies. In this study, integrated microarray data on pulpitis were used to obtain a list of normalized differentially expressed (DE) genes for analyzing the molecular mechanisms underlying pulpitis and identifying potential diagnostic biomarkers.MethodsA systematic search of public microarray and sequencing databases was performed to obtain expression data of pulpitis. Robust rank aggregation (RRA) was used to obtain DE gene lists (RRA_DEmRNAs and RRA_DElncRNAs) between inflamed pulp and normal samples. DE genes were evaluated by functional enrichment analyses, correlation analyses for inflammation-related RRA_DEmRNAs, and protein-protein interaction and competing endogenous RNA network construction. Quantitative real-time polymerase chain reaction validation was applied in snap-frozen pulp tissues.ResultsUsing the GSE77459 and GSE92681 data sets, 280 RRA_DEmRNAs and 90 RRA_DElncRNAs were identified. RRA_DEmRNAs were significantly enriched in inflammation-related biological processes and osteoclast differentiation and tumor necrosis factor, chemokine, and B-cell receptor signaling pathways. The molecular complex detection and cytoHubba methods identified 2 clusters and 10 hub genes in the protein-protein interaction network. The competing endogenous RNA network was composed of 2 long noncoding RNAs (ADAMTS9-AS2 and LINC00290), 2 microRNAs (hsa-miR-30a-5p and hsa-miR-128-3p), and 3 messenger RNAs (ABCA1, FBLN5, and SOCS3). The expression between most top inflammation-related RRA_DEmRNAs in pulpitis showed positive correlations. Quantitative real-time polymerase chain reacation validated the expression trends of selected genes, including ITGAX, TREM1, CD86, FCGR2A, ADAMTS9-AS2, LINC00290, hsa-miR-30a-5p, hsa-miR-128-3p, RASGRP3, IL3RA, CCDC178, CRISPLD1, LINC01857, AC007991.2, ARHGEF26-AS1, and AL021408.1.ConclusionsThe identified biomarkers provide insight into the pathology and may aid in the molecular diagnosis of pulpitis.  相似文献
